The Senior Division provides countywide programs in San Francisco and Alameda, helping older adults remain in the community and age with dignity, security, and a high quality of life. Program Description
The Ombudsman Office receives a mandated reports of abuse and neglect from various mandated reporter per WIC 15630.
This Abuse Specialist will receive Ombudsman Certification training and carry out the duties in accordance with Federal and State of California Law

Job Description
This position requires the Abuse specialist to investigate abuse reports involving health and safety and harm, directly and to work with the assigned ombudsman to the facility where the victim resides. This position is that of responder to abuse reports involving harm to the victim and as team leader for those assigned Ombudsman going already visiting facilities according to a coverage plan.
The Program needs a staff person to monitor incoming number abuse reports , for victims of abuse in different licensed facilities. This includes skilled nursing,, residential care facilities elderly( both board and care, and assisted living); as well as Adult Residential Care Facilities.
This person will report to Program Director and Co-Director of the Ombudsman Program

Job Duties:
  • To review received abuse reports and arrange by type of abuse
  • In investigate allegations, conducting face to face with victim
  • To liaise with Adult Protective Services, when there is a cross report
  • To review incoming abuse reports and sort by urgency and response.
  • To respond directly to abuse reports involving dependent adults and triage with with case managers and agencies who have either Developmentally Disabled or Behavioral Health Clients.
  • To attend city wide MDT meetings through Institute of Aging
  • To work with Program Supervisor to conduct in-services on mandated reporting
  • With Program Co- Director, attend Forensic Team and EDRT Meeting as needed
  • To advocate for client autonomy in the context of protection measures.
  • To coordinate case- resolutions with Community Care Licensing and relevant case management agencies
Requisite Skills:

  • Ability to conduct abuse investigations in a factual and objective manner
  • Good communication skills geared towards persons with medical or disability history.
  • Skills in case-coordination, and problem solving
  • Critical thinking and analytic skills desirable
  • Knowledge of the various abuse related agencies
  • Able to manage tasks and work flow of Program Ombudsmen
  • Knowledge of Microsoft platforms.
Qualifications:
  • Preferably Masters Level in Social Work, Public and Private Health Systems. With knowledge of aging and disability processes. Prior Ombudsman experience optimal.
