Job Duties (including, but not limited to):

• Evaluating and assessing Participant’s mobility level and other functional abilities
• Coordination of occupational therapy services with the LIFE Interdisciplinary Team and Care Plan Team
• Conducting initial and ongoing assessments and functional status
• Develops treatment programs when indicated
• Sets goals for Participants
• Provides education to Participant and family as necessary
• Completes all required documentation of services
• Performing health histories and exams prior to enrollment and scheduled assessments thereafter

Employment Requirements:

• Possession of valid Driver’s License and access to own vehicle
• Successful completion of Pennsylvania Criminal History Check, PA DHS Child Abuse Clearance, Office of Inspector General check, and FBI Fingerprinting for Child Protective Services Law
• Successful completion of post-offer employment drug screening and physical
• CPR and First Aid Certification, or willing to obtain at employer’s expense
• Excellent communication and organizational skills with attention to detail
