Basic Function:
The primary purpose of your job position is to conduct and coordinate the development and completion of the resident assessments and plan of care to ensure the resident attains his/her maximum level of well-being in the most cost efficient manner from pre-admission through discharge.

Dimension:
As the Case Manager, you are delegated the administrative authority, responsibility and accountability necessary for carrying out your assigned duties.

Nature and Scope of Essential Job Functions and Performance Criteria:

Major Duties and Responsibilities: Conduct and coordinate the development and completion of the Resident Pre-Admission Assessment. Perform administrative duties such as completing forms, reports, evaluations, etc., as necessary. Develop, implement and maintain ongoing Quality Assurance Program. Assist resident and the interdisciplinary team in planning and implementing discharge plans. Participate in surveys made by authorized government agencies.

Committee Functions: Work with the interdisciplinary team in developing comprehensive resident assessments and care plans for each resident. Serve on and participate in and attend various other committees of the facility (quality improvement, utilization review, etc.) as required.

Personnel Functions: Maintain an effective, friendly working relationship with health professionals, physicians, consultants, and governmental agencies that may be involved in the resident assessment and plan of care. Meet with and solicit information from therapist, department supervisors and staff concerning the resident assessment, the resident’s progress and plan of care.

Nursing Functions: Conduct or coordinate the interviewing of each resident and/or their sponsor for their pre-assessment and assessments after admission. Work with facility directors and supervisors in scheduling the resident assessments and care plan meetings. Develop and implement procedures to inform all assessment team members, nursing staff, and other departments of the arrival of newly admitted residents. Identify goals during pre-admission assessment for each resident and begin discharge planning upon admission. Communicate with resident and/or sponsor throughout the stay, the resident’s progress toward outcome attainment, need for goal modification, and/or identity barrier to discharge that may affect resident’s eligibility for benefits.

Assessment Functions: Develop preliminary assessments of the nursing needs of each resident prior to admission. Based upon the pre-admission assessment, review the care and services each resident may require in order to determine if the resident is appropriate for admission to this facility. Work with the interdisciplinary team to develop a plan of care that returns the resident to their maximum level of well-being and in a timely and cost effective manner. Assist each department supervisor in ensuring that all personnel involved in providing care to the resident are aware of the resident’s needs and care plan. Working with the Patient Care Coordinator and Assessment Nurse, ensure that a complete and accurate resident assessment is completed as scheduled, i.e. Day 5, Day 14, Day 30, Day 60, Day 90, or when therapy is terminated. Provide required information to the accounting office each time an assessment is completed. Review resident’s therapy treatment with each therapist daily.

Resident Rights: Maintain the confidentiality of all resident information. Ensure that all residents are treated fairly and with kindness, dignity and respect. Ensure that all resident interviews are conducted in private. Review any complaints or grievances made by residents and ensure they are reported to the appropriate supervisor and the Administrator. Ensure that the facility’s policies and procedures governing advance directives are reviewed with the resident and/or sponsor.

Qualifications:
Education/Experience: Must possess a professional degree from an accredited college or university that can be related to the requirements of the position. Licensed as an LPN/RN in the state of Ohio with 2 years Case Management experience. Knowledge of the Long-Term Care Survey process.

Physical and Sensory Requirements: Must be able to move intermittently throughout the workday. Must possess sight/hearing senses or use prosthetics to function adequately in fully meeting the requirements of the job. Must be in general good health and able to cope with the mental and emotional stress of the position.

Specific Requirements: Must be able to read, write, speak, and understand the English language. Must possess the ability to make good independent decisions when circumstances warrant such action. Must possess the ability to deal patiently and tactfully with residents, family members, personnel, visitors and the general public. Must possess the ability to plan, organize, implement and prepare medical reports, assessment, etc., in a systematic, neat, and timely manner.

Working Conditions: Works throughout the facility, in an office, and travels to various hospitals. Sits, stands, bends, lifts and moves intermittently during working hours, is subject to frequent interruptions. Is involved and communicates with residents, family members, personnel, visitors, and the general public under all conditions/circumstances and who may be hostile and emotionally upset. Works beyond normal working hours, on weekends, evenings, and holidays when necessary. Is subject to falls, odors, exposure to infectious waste, disease, conditions, etc.
