El Dorado Community Health Centers, a FQHC (Federally Qualified Healthcare Center) offering Primary Care Services with locations in Placerville and Cameron Park, is seeking a full-time Support Services Coordinator for their Mobile Medical Unit. As a non-profit community health center EDCHC provides Medical, Dental, Optical, Pharmacy, Behavioral Health and SUD Treatment Services to underserved populations.

Job Summary: As a member of the Mobile Medical Unit team, the Mobile Medical Unit, Support Services Coordinator (MMU SSC) serves as a resource for community partners and EDCHC medical staff serving patients from the MMU. They act as a source of contact for both new and established patients receiving services and are responsible for program management activities. The MMU SSC enables the staff supporting MMU to function as a collaborative care team, centered on the needs of the patients in the field. This position will work to monitor, manage, and access services for patients, as well as assist with the coordination and referral to receiving services on the MMU. Responsible for determining and assessing client support needs at various field locations, addressing barriers to care and connecting clients to the MMU for medical services. Facilitates client access to community resources and agencies, assists with specific provider requests as appropriate to MMU services and includes providers in client progress and/or resolution of requests for assistance.

The MMU SSC also provides administrative support to the medical department with scheduling patients and the tracking of appointment data. On a regular basis, this position will exercise independent judgement in the resolution of administrative problems, with the ability to manage multiple projects simultaneously while maintaining an overview of community partnerships relating to the MMU operations and day-to-day clinical schedule.

Experience & Educational Requirements:

  • One-year minimum of medical office experience with scheduling and administrative duties, which included organization and coordination at progressively responsible levels.
  • Experience within a Federally Qualified Health Center (FQHC), Community-Based, health or social services environment, and project management is highly desirable.
  • High school diploma or GED required. Two or more years of college preferred.
  • Bilingual Spanish/English preferred.
  • Ability to obtain CPR Certification.
  • CMSP Path to Health and CMSP Connect to Care Enrollment Training preferred.
  • Must possess a valid California driver’s license, have reliable transportation, a clean driving record and insurance as required by the state.
