El Dorado Community Health Centers (EDCHC), with locations in Placerville and Cameron Park, is seeking a full-time RN Case Manager for the Mobile Medical Unit (MMU).

Under the direction of the Mobile Medical Provider Lead the MMU RN Case Manager will support the operational activities of the mobile medical unit. The MMU RN Case Manager is responsible for coordinating nursing services at MMU sites and any related field activities ensuring nursing services are delivered within scope of RN licensure and protocols. Responsible for assessing, developing, implementing, coordinating, monitoring, and evaluating program services across the care continuum. Nursing services provided are coordinated in conjunction with the EDCHC Chief Medical Officer and MMU Provider Lead, to ensure integration with all Center service delivery goals.

EDCHC is a 501c non-profit with a HPSA score of 18. EDCHC is an eligible employer for Public Loan Service Forgiveness.

Education & Licensing Requirements:

• Associate or BS Nursing degree.
• Active California RN License in good standing.
• Current AHA BLS/CPR Certification required.
• Certification in Wound Care required.

Other Qualifications:

• 2+ years experience as an RN in any setting.
• Experience with nursing care coordination/case management.
• Experience in a community clinic or primary care setting preferred.
• Familiarity or proficiency in using eCW Electronic Health Records desired.
• Bilingual in Spanish highly desired.
