This position is eligible for a sign on bonus up to $2,500 with a 2-year commitment to Mission Health. Sign-on bonus will be paid out in your first paycheck!

Job Summary and Qualifications

The Medical Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. The social worker acts as a liaison between the patient and community resources/post-acute care providers. The social worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The social worker facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput.

What qualifications you will need:

Required Education: Bachelor of Social Work (BSW)

Preferred Education: Master of Social Work (MSW)

Required Certification for Level II: ACM-SW or CCM Required.

Required License: State licensure preferred (may be required by state regulations)

Preferred License: Certification in social work or case management preferred

Required Experience: Knowledgeable about internal, community and post-acute care resources. Basic social worker skills including but not limited to empathy, sensitivity to cultural and ethnic diversity, advocacy, communication and crisis intervention.

Required Experience for Level II: 3 years hospital medical social work experience

Preferred Experience: Acute care social work experience preferred
