The Role:

To increase Offsite/ Remote SUD services as evidenced by increases noted in SUD services, revenue, expansion sites, public exposure and most of all positive outcomes for individuals, families and communities.


Responsibilities:

KEY RESPONSIBILITIES AND ACCOUNTABILITIES

Overall Offsite Ambulatory Services Program Management

  • Develop feasible community based partnerships and establish initial clinical, peer, and administrative services and support as necessary
  • Provide SMART business objectives with trackable, measurable goals in support of building offsite service delivery system
  • Initiate and oversee initial low threshold engagement groups and services, with intention of building fiscally and clinically viable teams for eventual turnover to coordinators for specified sites and billable provider teams
  • Schedule, regular meetings to focus on marketing and admissions, increased services, manage case conferences and reduce challenges, building strength-based approaches and support regulatory compliance.
  • Maintain relationships with multiple community-based organizations and build out network model collaborations around them that complement our services, and fills needs they may have to make recovery and change easier and more attractive to their respective clients and staff.
  • Provide administrative oversight of the off-site models of care, including developing this system to align and integrate into our CASN, Cogency and new potential HEROs or other regional organizational models to allow steady payer mixes and negotiated rates for Managed Care Medicaid and commercial insurance contracts
  • Increase census by establishing relationships and agreements for services with inviting and welcoming organizations around the city. This will include maintenance of benchmarked units of service and financial feasibility.
  • Build a diverse portfolio that will include, but not be limited to Homeless Shelters, Family networks, alternative and public schools, recovery and problem solving courts with a goal of starting up a new site every 6 -8weeks or as strategically planned with CRW executive management, within fiscal formula models in accordance with our CAO/CFO and ED.
  • Assist as needed with Project Contact (PCO) by providing individual and group therapy and completing detailed progress notes, treatment plans, assessments, and other documentation required by OASAS and CARF standards.


Requirements:

  • Qualified Health Professional (LCSW, LMHC, CASAC) requirement
  • Experience in creating offsite programs for substance abuse services
  • Experience in the administration of shelter-based and community-model programs
  • Experience working with SUD and Co-occurring mental health population
  • Ability to maintain high ethical and moral standards, including systems development for providing trauma-informed supervision for our certified peers and licensed providers so vicarious trauma does not trigger our staff
  • Ability to work collaboratively and manage multiple priorities and projects effectively
  • Ability to handle stressful situations in a professional, courteous and friendly manner
  • Excellent interpersonal, organizational, verbal, and written communication skills
  • Self-motivated, strong attention to detail, and highly analytical
  • Ability to work autonomously and meet benchmarked goals and deliverables without a micro-management focus, and the ability to be a self-starter and take healthy positive risk without placing the overall agency in compromising situations per our policies, protocols and filed standards
  • Strong information technology skills and well-versed in Microsoft office products; including the development of risk management grids, benchmark goal monitoring reports and fiscal and clinical compliance tools in collaboration with our facility and overall agency compliance departments

Annual renewal protocols:

  • This position will be funded through our third-party managed Medicaid reimbursement strategies.
  • The position will be reviewed annually to assess for both clinical and fiscal compliance and sustainability.
  • In the event the program is not monetarily sustainable, Educational Alliance, Inc/ CRW maintains the capacity to freeze or terminate the continuance of said programming and this position. This will be reviewed at the end of each Funding Year and in the event a sustainable business model is not possible we reserve the right to review for non-continuance of said services and this position.
  • Additionally, given this is a third-party revenue generator, EA/CRW will review annually for a proactive salary adjustment percentage for each new site that is developed and holds a sustainability level for both clinical and fiscal goals for an agreed upon number of months (site dependent and individualized).
  • The Manager of Off-Site Ambulatory Services must agree to these terms and conditions and will be asked to provide corrective action and strategic planning options to assure all sites developed remain fully compliant, operational and sustainable.
  • The position will receive a $4,000.00 bonus to their overall base salary for every offsite center that opens for Center for Recovery and Wellness.
