LPN (Licensed Practical Nurse) Job at Horizon Medical Group, PC
Horizon Medical Group is seeking a Full -time or Part-time LPN for multi-provider family practice located in Orange County, New York.
The ideal candidate must be dependable,motivated and self-directed. Must have good decision-making skills, confidence in telehealth communication. Must be able to multi-task and provide value in a team setting. Nights and/or weekends optional.
Some duties include varied triage patient calls for multi-provider practice. Must be able to accurately relay information to providers via verbal or written communication, as well as accurately educate patients on importance of visit compliance, follow-up visits,medication adherence,as well as identifying acute medical needs. A positive caring attitude and ability to work well with others a must.
Hours Monday - Friday with possibility of nights and weekends
Qualifications:
Must have a valid LPN license, 1 year nursing expirence, BLS certifcation, Home care expirence.
