At Nucleus Healthcare, we attract, and nurture mission-critical talent needed to create a diverse pool of healthcare providers. Our client, located in Sellersville, PA, is seeking passionate, dedicated candidates for a Licensed Practical Nurse (LPN) position.

  • Shifts: 3pm - 11pm, 40 hours per week. Alternating weekends
  • Pay: $33/hour ($1 shift differential for 2nd shift)

The ideal Licensed Practical Nurse (LPN) candidate has experience caring for residents with dementia and working in long-term care.

Licensed Practical Nurse (LPN) Responsibilities:

  • Promote residents’ well-being and satisfaction by supporting resident-centered care, developing/interpreting standards of care, guiding the interdisciplinary team as it establishes and implements the residents’ plan of care
  • Communicate with other household members regarding changes in resident care plans, diet changes, etc.
  • Participates in/provides input for resident care conferences
  • Ensure medical treatment is provided in a manner that honors resident preferences and individuality; demonstrate commitment to the highest clinical outcomes
  • Administer medications/perform treatments and nursing procedures according to established policies & procedures and nursing standards of care
  • Ensure that all physician/consultant orders and changes to orders are followed
  • Ensure narcotic records are accurate for the shift and notify the Household Nurse Leader and/or Director of Nursing of all drug and narcotic discrepancies noted
  • Ensures an adequate quantity of supplies readily available to personnel and that resident’s bedrooms are maintained in a clean, safe and sanitary manner
  • Administer professional services such as catheterization, intravenous therapy, tube feedings, suction, applying and changing dressing/bandages, packs, colostomy and drainage bags, range of motion exercises, care for the dying/dead, obtaining sputum, urine and other specimens for lab tests, etc..
  • Monitor activities of daily living of residents in relation to the normal aging and/or disease process through assessments of mental status, physical condition and emotional and spiritual well-being
  • Document assessments and progress notes in the EMR in an informative and descriptive manner
  • Perform other administrative duties such as completing medical forms evaluations, studies, reports, etc. and any required filing
  • Plan, develop, implement and evaluate the activity/recreational programs of the household; seeks out opportunities for enhancing residents’ life in the household and community

Education & Experience:

  • Nursing Degree from an accredited college or university, or be a graduate of an approved LPN training program
  • Active Practical Nursing license in the state of Pennsylvania
  • Nursing experience in a hospital, acute skilled nursing care facility, or other related medical facility
  • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care
  • Must be willing to work harmoniously with other personnel and families

Job Types: Full-time, Contract

Pay: $33.00 - $34.00 per hour
