POSITION SUMMARY:

The Hospice Licensed Practical Nurse (LPN) will provide skilled nursing care to patients in accordance with physician’s orders, directed by the RN, and in compliance with Agency policies/procedures, State and Federal regulations.

ESSENTIAL FUNCTIONS: To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation; including but not limited to:

• Provide nursing care and prescribed treatments for patients in accordance with physician’s orders and in compliance with company policies/procedures, state and federal regulations
• Implement the patient’s individualized plan of care, under the direction of the Registered Nurse, in compliance with company policies/procedures, state and federal regulations
• Participate in on-call rotation
• Willing to travel to other office locations to assist with staffing as needed
• Maintain appropriate documentation of care provided to patient and any communication with the patients’ physician and patient representative(s)
• May provide personal, and hygienic care for patients

Required Skills:

• Work requires advance knowledge of the Homecare Industry, Medicare Regulations, CMS, and other State/Federal regulatory changes

Required Work Experience:

• One (1) year experience as a Licensed Practical Nurse preferred
• One (1) year home health experience preferred

Licensing/Certification/Education Requirements:

• Graduate of an accredited Practical Nursing (LPN) Program
• Current License Practical Nurse (LPN) license
• Current CPR Certification

Other Requirements:

• Position may require unscheduled overtime, week-end work
• Must be willing and able to work flexible hours
• Travel required

Physical Requirements:

• Job requires frequent lifting of objects up to 40 pounds
