Schedule: Full Time
Days/Hours: Monday through Friday - 8:30am to 5:00pm
Pay: $26.98 - $33.69

Responsibilities:
SUMMARY:The Licensed Practical Nurse (LPN) provides support to patients and providers in conjunction with Registered Nurses (RN) under the supervision of the Practice.
Job Duties & Essential Functions:
Accurately collects clinical data on patients and demonstrates competency in reporting abnormalities to Medical Provider.- Field patient phone calls and troubleshoot medication management issues.
- Process medication renewals and complete prior authorizations as needed.
- Documents nursing observations, interventions, and patient’s response to treatment in patient records.
- Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals in collaboration with the Registered Nurse (RN).
- Maintains standard precautions in the provision of patient care.
- Demonstrates competency in answering phones, documenting clear, concise messages in EMR, providing accurate information to patients and providers, retrieval of reports.
- Keeps records of all laboratory work sent, files laboratory reports on client record, and prepares and processes Lab/Diagnostic requisitions.
- Conforms to policies and regulations governing patient rights and confidentiality of information.
- Adheres to protocol for patient message workflow.
- Perform all other related duties as assigned by management.
Required Education & Qualifications:
Graduate of an accredited school of Practical Nursing.- Currently registered in New York State as a Licensed Practical Nurse (LPN).
- Must have excellent communications skills
- Six months or more of experience working in a high volume ambulatory care setting answering phones, fielding messages via EMR
- Three years’ experience in ambulatory care setting required as a LPN
