The Loudonville Assisted Living Residence (LALR) is a well-respected, long-standing, family-owned and operated assisted living community located in picturesque Loudonville, NY.

LALR is seeking a collaborative and compassionate, licensed practical nurse (LPN) to be our Coordinator of Resident Care.

The Resident Care Coordinator plays an integral role in LALR's success at providing the highest level of quality and compassionate care. You will work closely with our owners, executive director, and department heads to ensure LALR’s assisted living residents receive the care they need as they age-in-place throughout one of our three centers; assisted living, enhanced assisted living and memory care.

Resident Care Coordinator (RCC)/Licensed Practical Nurse (LPN) Responsibilities

  • Responsible for the development, organization, and operation of nursing and supportive services for the Assisted Living Center.
  • Orient and assist all residents to adjusting to life in the community.
  • Train, supervise, schedule, review and discipline care team staff including, LPN’s, medication aids and resident aids.
  • Coordinate and monitor all services relating to resident care needs through individualized service plans.
  • Address resident and family concerns in a timely and appropriate manner.
  • Model leadership and maturity to staff; present professionally to residents and family members.
  • Direct Case management for residents.
  • Makes daily rounds on unit to supervise, observe, examine, and interview residents to monitor delivery of care and regulatory compliance.
  • Ensures 24-hour availability as on-call.

Resident Care Coordinator (RCC)/Licensed Practical Nurse (LPN) Qualifications

  • Graduate of an accredited school of nursing. Unrestricted nursing license to practice in New York (LPN, RN).
  • Demonstrated experience supervising team members and providing training and support.
  • Dedicated to the principles of kindness, compassion, service, and excellence.
  • 2 years-experience in assisted living/long-term care preferred.

Additional Resident Care Coordinator (RCD)/Licensed Practical Nurse (LPN) Benefits

  • starting salary of $70,000
  • 3 weeks of paid time off in year one of employment-(5-days) paid vacation upon acceptance and an accrual of 40 hours paid vacation, and 40 hours PTO within your first year. In year two this increases to 4 weeks and so on.
  • annual increases of 3-5%.
