The LaSalle School is seeking a Lead Case Manager (Full-Time, 40/hours per week, non-exempt/hourly).

Our ideal candidate is energetic case manager with knowledge of and experience with trauma informed treatment, evidence-based practices, able to assess youth needs and identifing ptotentional youth sponsors. Strong computer skills: strong management, leadership, organizational and time management skills, excellent written and verbal communication skills, efficient documentation and record keeping skills, able to work effectively and collaboratively within a highly diverse multidisciplinary team of service providers and referral agencies; knowledge of all relevant current, and emerging, regulatory requirements (e.g., ORR, etc.)

GENERAL STATEMENT OF DUTIES:

The Lead Case Manager is responsible for coordinating and providing case management and safe and timely release services to children in LSS care, training new case managers, and supervising the work of case managers. Also, to ensure all program services are provided in accordance with applicable LSS, OCFS, and ORR Policy and Procedures, as well as being responsible for the maintenance of the UC Portal.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

General Responsibilities/Duties:

  • Provides daily oversight to the case management team to include, but not limited to, guidance related to service delivery and documentation, maintaining compliance with LSS, OCFS, and ORR standards, appropriately managing caseloads, and coordinating on-call scheduling.
  • Responsible for working with the child and the child's family/sponsor. This includes locating family members, assessing potential sponsors to unify the child with their families/sponsor, conducting background checks, and other tasks to promote reunification.
  • Assess the needs of each child in care, while developing an Individual Service Plan, and facilitate a safe timely release/discharge, in collaboration with the Clinician.
  • Plans with the Clinician for conferences, phone calls, and family visits which are to be coordinated with childcare according to team plans.
  • Responsible for maintaining case files on his/her caseload and document the provision of services in each child's case file while maintaining ORR/UC database compliance.
  • Responsible for completing initial intake, assessments, case reviews, and case notes, to meet program and ORR deadlines.
  • Responsible for the facilitation of the preliminary Casey Life Skills and other identified assessment tools.
  • Provides transportation and supervision of UC to medical, legal, and other appointments as assigned.
  • Coordinates the discharge or transfer of each child including post-release services as indicated.
  • Report and discuss cases with third party reviewers, clinical staff, and the Federal Field Specialist
  • After hours on-call duties, as assigned.
  • Other duties as assigned by supervisor.

EDUCATION and/or EXPERIENCE:

Required:

  • Bachelor’s degree in Psychology, Human Services or other related field with 3+ years of supervisory and case management experience in Human Services.
  • Basic Knowledge of English and Spanish language and writing, ability to learn and become bilingual. (Training will be provided)
  • New York State Drivers License in good standing as determined by the Agency.

Preferred:

  • Master's Degree in Psychology, Human Services or other related field.
  • Bilingual in English and Spanish speaking and writing.
  • Case Management experience within the Child Welfare system.

Wage & Benefits:

Hourly Wage: $19.23-$24.04

Paid Time Off:

  • Start at 10 days off for vacation, 13 paid holidays a year, 3 personal days, 12 sick days per year and other leave time available dependent on the circumstances.
