Position Overview:

The Arc is seeking a Service Coordinator to receives referrals; contacts family; provides information regarding program to parents and/or guardians. Assesses the child’s needs and family needs. Arranges for developmental assessments as needed. Coordinates between school personnel, family and other relevant agencies and individuals to develop the child’s IFSP. Assists in the implementation of the IFSP. Provides resources and linkage services to meet the goals stated in the IFSP. Monitors meeting the goals in the IFSP. Advocates for the child and family. Educates parents and/or guardians as to their legal rights under the Infants and Toddlers Program.


Key Responsibilities
:

  • Completes home visits and corresponding assessment of children and family’s strengths, resources and needs. Obtains reports of previous medical and developmental evaluations.
  • Collaborates with school personnel and family to obtain assessment of child. Arranges transportation to assessment when necessary.
  • Follows medical assistance regulations for documentation for ITP targeted case management.
  • Maintains record in approved ITP record format.
  • Attends in-services and other appropriate programs to enhance knowledge of skills needed for service coordination in the ITP.
  • Attend ongoing training arranged by PGCPS.
  • Expected to travel throughout county for meetings.
  • Assists in the implementation of the IFSP in the following meetings: Six Month Review, Annual
  • Review, 24 Month Goal, Transition Planning Meeting and IEP Eligibility Meeting.
  • Provides resources, referrals and linkage services to meet the needs of the child and family.
  • Appropriately interacts with clients and professionals while in attendance of meetings/workshops.
  • Demonstrates understanding of ITP regulations by providing proper guidance and direction to families.
Requirements:

Must-have Qualifications:

BA or BS Degree in a related field. Will consider the parent of a child with a disability with experience in providing case management to families with children birth to 3 years of age in lieu of degree. Must have excellent oral and written communication skills.
