Outpatient practice in the beautiful Prairie Avenue Health Center in Lower South Providence neighborhood. On-call duty is phone-based and shared with a large group; Providence Community Health Centers, Inc. is an innovative, federally qualified health center. - This is a neighborhood-based clinic, NCQA Level 3 patient-centered medical home, serving a multicultural patient population
- Providers have dedicated RNs and dedicated bilingual Medical Assistants
