Purpose:

Assists families in connecting with community and state-based child developmental services, hearing screen referrals, resources, and prevention services to ensure healthy development for families and their children.


Responsibilities:

  • Receives and processes referral and intake information from outreach staff, nurses, case managers, hospitals, social service agencies, Early ACCESS and Iowa Family Support Network through phone calls, walk-ins, and the community.
  • Assists with referral assessments through phone calls, walk-ins, or in the community for EveryStep programs including Healthy Start/Empowerment, Community Home Visitation, Strong Foundations, Family Support and Group based services, Early ACCESS, Home Care and Nurse Family Partnership.
  • Collects and documents accurate and timely program data, care coordination information and documentation of program activities in the EveryStep and Early ACCESS databases within required time frame.
  • Assists and maintains up-to-date directory of community-based family and child health organizations, programs, services and resources.
  • Serves as the point of contact for referring primary care providers and families to link children to intervention services related to developmental screening, developmental delays, and healthy mental development concerns and provide follow up to referring primary care practices regarding referral activities and status.
  • Assists with a 24 hour web based social media outlet to promote public awareness to include Pinterest and Facebook, in addition to Parentivity, Virtual Home Visitor, You Tube, Instagram and other emerging social media.
  • Displays a cooperative relationship with staff, contracted agencies, case managers, outreach workers, interpreters and supervisors which also includes local and state agencies in relation to referrals, assignments, transfers, data entry and assessments.
  • Assists with Affordable Care Act calls for the Navigation Grant. Complete scheduling requirements for navigation appointments and assess needs and resources on an as needed basis.
  • Assists with referrals for mental health and depression concerns from OB/GYN clinics for Healthy Start and CHV.
  • Conducts assessments for appropriate programming and resources, conducts follow up calls.
  • Attends monthly Coordinated Intake and Case Manager Task Force meetings, weekly Intake meetings, and quarterly Healthy Start Consortium meetings.
  • Attends monthly Coordinated Intake, Healthy Start Team, and Case Manager Task Force meetings and quarterly Healthy Start Consortium meetings.
  • Completes mandated and required pre and in-service trainings.

Qualifications:

  • Bachelor’s degree in related field strongly preferred
  • Minimum of two years’ experience in social services, case management, intake/referral services and/or maternal/child health
  • Strongly prefer Bi-Lingual English/Spanish
  • Must be able to communicate effectively in verbal and written English
  • Aptitude for technical writing and analytical skills to interpret information
  • Knowledge of database use and management and prefer experience with Microsoft Word and Excel as a minimum
  • Experience working with diverse communities preferred
  • Knowledge of community and state resources
  • Ability to identify family strengths and needs for family-centered support
  • Ability to exercise effective independent judgment and problem-solving
  • Ability to work as a member of a high performance team
  • Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire)
  • Must have valid driver's license, auto liability insurance and reliable transportation if driving is required
  • Successful completion of pre-employment physical, drug screen, 2-step TB test and background check

  • Time: Ability to be flexible with a varying schedule and programmatic requirements
  • Travel: May be required between locations
