This position is responsible for aiding in locating suitable housing and supporting candidates seeking employment for persons in Delphi Rise’s Home Safe program. The Home Safe program is a 40-unit, scattered site, Rapid Rehousing Project of the Rochester Continuum of Care and Housing and Urban Development. We are looking for a team-player with a proven record of client engagement. Diverse candidates are encouraged to apply.

Job Responsibilities:

1. Understand the challenges of those individuals’ seeking services who are returning into society after prolonged incarceration

2. Assist program participants to locate sustainable housing

3. Assist candidates in securing and maintaining employment

4. Carry a caseload of approximately 20-25 clients

5. Maintain contact (via phone/ Zoom/ In person) with participants on caseload on a regular basis

6. Conduct home visits and annual assessments for program participants

7. Coordinate participant’s services with Delphi Rise Care Services and Outpatient Departments

8. Serve as a liaison for Health Homes and Reentry referrals and participate in monthly Care Services Case Reviews

9. Provide direct case management for those participants ineligible for case management through other community programs

10. Maintain accurate and timely records in accordance with program requirements utilizing HMIS system

11. Perform other tasks as assigned

Qualifications:

We are looking for a candidate to possess a combination of skills, experience, and education. However, for numbers 1 through 5 below the candidate only needs to have at least three of the criteria listed to be considered. Criteria 6 and 7 are required.

1. An Associates or Bachelor’s degree in social work, criminal justice, addiction, psychology, counseling, or a closely related field.

2. Experience working with clients with criminal justice involvement, mental illness, homelessness and/or addiction

3. Case management experience

4. Previous experience as a job coach or in recruitment

5. Familiarity with community resources and a track record of successfully accessing resources for client benefit

6. A commitment to working as part of a multi-cultural staff to provide appropriate services to a widely diverse client population is required

7. Reliable transportation and a clean license required
