Hospice RN Case Manager Job at St. Paul Elder Services, Inc
Hospice RN Case Manager is a part of a collaborative, interdisciplinary team who provides high-quality, person-centered end-of-life services/support including those living at our Kaukauna and Green Bay campuses as well as those living in a 5 county territory. The Registered Nurse Case Manager works collaboratively with patients, clinical teams, and family members to ensure physical comfort and symptom management based on individualized assessments and patient-specific needs and goals.
Candidates with experience in hospice, palliative, or aging service preferred and committed to embracing St. Paul Elder Services' core values of dignity, compassion, respect, hospitality, and stewardship. Must be a licensed Registered Nurse in the state of Wisconsin.
Job Types: Full-time, Part-time
Pay: $30.00 - $35.00 per hour
