Hospice Nurse Job Description:
Embracing HospiceCare is seeking a highly motivated and energetic Hospice Nurse Case Manager with at least 1year of hospice or home health experience to work within Mercer/Middlesex Counties.
The Hospice Nurse Case Manager establishes a professional and caring relationship with our patients and families, support staff, physicians, discharge planners, social workers, facilities and other healthcare professionals by providing hands-on care, on-going clinical management and education regarding hospice care.

Job Requirements:
- Registered Nurse with a current NJ RN license
- Covid Vaccination + Booster ( Will accept with a Medical or Religious Exemption)
- Current vehicle liability insurance and license
- Excellent people, verbal and written communication skills
- Ability to adjust to daily needs of patients
