Who we are... Partners In Home Care is the premier home health agency in Montana. With a staff complement of 200 strong, we serve a radius of 100 miles around Missoula with a broad array of professional healthcare including nursing, occupational therapy, physical therapy, social work and bereavement support. Our programs encompass Home Health, Hospice (the largest Hospice in the community), Home Infusion, Home Medical Equipment (including home modifications for those who want to age-in-place) and HCBS (the Medicaid “waiver” program that supports those who wish to live with independence).

This is a part-time position (.6FTE is 24 hours per week) and has flexible schedules. Opportunity to work from home with scheduled in-person office meetings. Candidates are required to live in our service area (Missoula, Montana).

What You will be Doing... The Hospice Bereavement Coordinator meets the bereavement needs of hospice patients, their families and the community at large.

Essential Duties and Responsibilities:
The Bereavement Coordinator develops and maintains the hospice bereavement program, coordinates grief and bereavement support for hospice patients and their families and provides short-term bereavement counseling for patients, their families and the community at large as needed and/or requested.

You will schedule regular bereavement support groups for hospice families and the community at large, interpret grief and bereavement factors that may affect patients and families for the interdisciplinary team (IDT), and you will serve as a resource for the Interdisciplinary Team's plan of care.

In addition to providing individual and community support the Bereavement Coordinator will provide bereavement training and grief support for hospice staff and volunteers. You also educate the the community at large with regards to bereavement issues. At times, you may provide referrals to appropriate community resources for staff, patients and families to access.

Your Qualifications... include a Bachelor’s degree with advanced training/education (advance degree in health or social sciences preferred); at least one year direct experience with death and grief issues; working knowledge of community resources and services; valid driver’s license; appropriate auto insurance and reliable transportation.
