Job Summary:
The Home Care Coordinator/Discharge Planning (HCC/DCP) is a registered nurse who has a thorough understanding of available home care services and products, eligibility requirements, and resources available in the community and manages the functions within designated hospitals. The HCC DCP acts as a discharge planning resource to Hospital staff involved in home care planning, and facilitates continuity of care for the patient referred to receive home care services. The HCC DCP responsibilities include overseeing day-to-day operations and supervision of staff, while also ensuring adequate staffing at designated sites.

Requirements:
Graduate of accredited RN program required; B.S. in Nursing preferred
RN license in the state of Maryland
Health Care Provider CPR: AHA or American Red Cross
Two (2) years of home care experience required with progressive care coordination experience.
