Contract: 13-weeks Full-time, 40 Hours GuaranteedTypical hours: 8 am to 5 pm Monday-FridaySetting: Field staff all work remotely after training. Our patients will be in the Chicago area specific to the location zone (NW, NE, SW, SE) of the Chicago area.
The purpose of the RN Case Manager:

• arranging necessary health-care services for patients requiring nursing services in their home
• Evaluating and executing appropriate treatment for referred patients with emphasis on quality of care, continuity of services, and cost effectiveness
• Conducts initial assessment and establishes a plan of care for patients and provides direct patient care

Requirements: Requirements:

• Active RN in IL
• Active BLS
• At least 2 years Experience (Hospice/Home Health)
• Covid and flu vaccination required
• Technically Savvy, Strong Computer and Phone Skills
• Must be willing to travel
• Strong EPIC experience required
• Reliable Transportation
• Active Driver's License
• Strong Communication Skills

Additional Job Responsibilities related to Home Hospital:

• Responsible for Caseloads of Patients in specialty areas.
• Conducting initial assessments to obtain detailed physical, and environmental data.
• Establishing plan of care in regulation with physician orders
• Advises and consults with family
• Provides interventions and on going assessments of patient care
• Completes documentation according to regulations

Job Types: Full-time, Temporary, Contract

Pay: $70.00 per hour

Schedule:

• 8 hour shift
• Monday to Friday
• Weekend availability

Work Location: On the road
