Position Summary

The Health Information Clerk is responsible for all routine functions provided by the health information department as assigned by the department manager, and doing so in a manner that is compliant with organizational and legal requirements for the protection of patient confidentiality.

Principal Duties and Responsibilities

  • Process medical records requests to and from LCHCin a timely and accurate manner, including the filing of correspondence, reports, and other paperwork in the patient chart that has been reviewed and initialed by the applicable provider.
  • Import clinical documents into patient charts in the EMR, including entry of any appropriate structured data based on requirements for each document type.
  • Process special requests by patients/staff/third parties by pulling and/or copying medical records for release or review.
  • Enter data into required systems outside of LCHC’s EMR to meet grant or other procedural requirements (Case Management systems like Cornerstone and RedCap; Care Management systems like MHN Connect).
  • Process faxes, messages, and other correspondence in accordance with applicable procedures.
  • Comply with organizational requirements regarding HIPAA, other medical records related regulation, and legal and accreditation requirements related to the confidentiality and disclosure of Protected Health Information (PHI).
  • Participate in department and ministry-wide staff meetings, and performs other duties as assigned.

Required Skills or Abilities

  • Ability to strongly embrace and personify the mission of the Lawndale Christian Health Center’ An ability to organize oneself, solve problems, and take initiative to accomplish assigned tasks.
  • Ability to collaborate and work under stress with a diverse group of health care professionals in a multi-cultural setting.
  • Computer skills for the purposes of tracking charts and logging work.

Required Knowledge, Education, Experience or Licensure/Registration

  • High school diploma or equivalent
