Position Overview

This position provides direct nursing care to patients in the IPU, their families, interdisciplinary group management, coordination of patient and family services, and evaluation of appropriate hospice care through development of the Hospice plan of care.

Essential Duties

• Assesses physical, environmental/safety needs of patients.
• Provides direct patient care according to the plan of care and in accordance with the Nurse Practice Act and Hospice Standards of Care.
• Develops and coordinates plan of care in collaboration with patient, significant others, and interdisciplinary care team members to focus on holistic needs of patient in order to assure satisfactory outcomes for patients.
• Provides in-patient facility nursing care according to the plan of care.
• Acts in a professional manner to collaborate with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals.
• Accurately and timely documents nursing assessments, interventions, and updates to Plan of Care according to the nursing standards of this organization.
• Supervises LPNs and nurse aides and documents supervision according to the organizational policy.
• Provides caregiver with appropriate instruction to support safe care of the patient in the IPU and in accordance with the current plan of care.
• Accurately utilizes the time clock.
• Complies with organization nursing standards.
• Maintains educational and communication requirements.
• Maintains confidentiality of patients and their families, co-workers, volunteers, and the organization at all times.
• Adheres to the organizations Standards of Conduct and HIPAA regulations.
• Reports any potential misconduct or violation of the organizations Standards of Conduct, HIPAA regulations, or federal or state laws and regulations.
