JOB SUMMARY: The Family Support Worker provides crisis intervention and other family support services to residents of the District of Columbia that include outreach, assessment, service planning, workshops, and linkage to internal and external community resources in the district. The position provides quality client social services in accordance with best practice standards, contract and grant requirements, and Agency practices and procedures.

ESSENTIAL DUTIES and REPONSIBILITIES:

  • Interview and assess clients for direct services and develop a case plan for the client including written short-term goals and tasks. Follow up with clients at 30/60/90 days if a client receives case management services.
  • Process requests for financial assistance including eligibility screening, intake and assistance with completing required documentation.
  • Assist clients in connecting to community resources including housing, job training programs, employment, and treatment for substance abuse and mental/physical health issues.
  • Plan and facilitate workshops.
  • Maintain complete and accurate case records to include entering data into the DHS/Agency databases within 24 hours of contact with clients. Complete required reports.
  • Keep supervisor informed of client matters requiring resolution.
  • Participate in all required Agency meetings, training and community meetings as required.
  • Perform other job-related duties as assigned.

EDUCATION and EXPERIENCE:

  • High school diploma or GED.
  • 2 years of work experience in a human services field.

SKILLS and COMPETENCIES:

  • Skill in the use of computers, preferably in a PC, or Windows-based operating environment.
  • Ability to communicate effectively in written and oral form, and ability to use good problem-solving techniques.
