SUMMARY OF DUTIES

The Family Support Specialist works with families regarding concrete issues that impact upon psychological or emotional problems such as stress, substance abuse, family situations or any other mental health issues that might be affecting the child’s ability to function successfully in both home, education and community settings. Implements each youth’s therapeutic treatment plan under direction of supervisor and therapist.

Job Type: Full-Time / Part-Time

RESPONSIBILITIES:

· Submits weekly billings to administrative office and prepares monthly authorization requests.

· Participates in all quality assurance activities, and interviews client to obtain information concerning medical history and other pertinent information to assist gathering information for the Clinical Assessment Forms.

· Prepares initial and updated Transistion to Adulthood Plans.

· Participates with other service providers, as appropriate, to accomplish joint services planning and to facilitate the provision of services.

· Provides assistance with training of providers to ensure appropriate instructional strategies, documentation and administrative requirements.

· Performs other duties as assigned.

REQUIREMENTS:

· Bachelor’s degree from an accredited college or university in psychology, social work, nursing, counseling, education, or special education, or

· Associates degree in psychology, social work, nursing, counseling, education, or special education with one year of experience

PHYSICAL REQUIREMENTS

· Observe, see, hear, and respond to client’s needs, emergencies and conflicts.

· Able to stoop, kneel, bend at the waist and reach on a daily basis.

· Able to frequently stand, sit and walk.

· Able to run after client occasionally.

· Be aware of the cognitive, social and physical needs of children.

· Handle the responsibilities and routine stress often associated with providing childcare services.

· Ability to communicate effectively (verbal and written); ability to instruct and maintain emotional control under
stress.

· Must be able to lift 50 lbs and carry a distance of at least 15 meters without stopping in 20 seconds or less.

· Individual will be exposed to inside and outside environmental conditions.

· The point of service delivery is primarily in the program site and community, but may include other locations, including, but not limited to client’s homes.

· If authorized by HBH to transport client (current driver’s license, clean traffic abstract and current car insurance)
