Title: Family Service Coordinator
Reports To: Building Principal, SCHRC Director of Children's Services

SUMMARY

As part of the Waseca County Collaborative for Families (WCCF) team, the Family Service Coordinator (FSC) provides individual and family services for children. The Family Service Coordinator serves as the liaison between school, family, and other community resources. The FSC provides services as needed in the school, home, and community setting. The focus of the FSC is on prevention and early intervention that is supported through employment placement within the elementary school setting. PRIMARY RESPONSIBILITIES
  • Provide individual and family services for children, serving as the liaison between school, family, and other community resources.
  • Provide services as needed in the school, home, and community setting.
  • Counsel and provide skills training to students individually and in small groups
  • Refer students to appropriate school and community resources/services.
  • Develop an ongoing working relationship with county and regional service providers.
  • Attend monthly FSC staff supervision meetings.
  • Develop a strong working relationship with school staff and assist in identifying, documenting and problem solving high-risk behavior patterns among students.
  • Inform and educate school staff on the FSC referral process for individual and group services.
  • Contact families of students who have been identified as at-risk and visit their homes as appropriate.
  • Counsel families of students who have been identified as at-risk on resources/services.
  • Refer family members to appropriate school and community services/resources.
  • Document referrals and services provided and track numbers for WCCF program evaluation.
  • Work with WCCF to bring about positive systemic change in the service delivery system in Waseca County.
  • Help develop prevention strategies and creative systems that will support at-risk students and their families.
  • Take in-service training courses as required for the position and maintain licensure when required.
  • Perform other duties as requested by the supervisor.
SECONDARY RESPONSIBILITIES

Align with the school's culture to become integrated within the school building. Perform miscellaneous projects and complete various tasks as requested by administration.

JOB SPECIFICATIONS

Education, Experience and Credentials

Bachelor’s degree in School Social Work, Social Work or Behavioral Science. Must pass a background check.

Knowledge, Skills, and Abilities
  • Excellent interpersonal communication skills; deals effectively with a variety of people and personalities; communicates clearly and effectively both orally and in writing.
  • Fluency in English and another language is a preferred qualification but not required.
  • Works as an effective and proactive team-player.
  • Able to work independently, showing initiative, and willingness to take charge of a situation.
  • Skilled in identifying and assessing a wide range of clients’ needs.
  • Knowledgeable about local and regional community resources and how to access them.
  • Applies effective time management skills to meet all deadlines.
  • Demonstrates thoroughness and organizational skills in the workplace.
  • Displays flexibility to meet the Center’s and clients’ needs in an environment with interruptions.
  • Shows proper etiquette when dealing with clients and other sources.
  • Familiarity with standard computer functions and applications.
  • Demonstrates the ability to organize meetings with clients, families, referral sources, etc. for consultation and collaboration.
  • Willingness to transport clients in their vehicle when necessary.
  • Understanding of funding resources for client services, including housing, special services, medical and mental health care, benefits assistance, etc.
  • Recognizes and understands mental health symptoms and diagnoses.
  • Experience in use of office equipment such as copying machine, fax machine, postage machine, paper shredder, programmable telephone system, e-mail, etc.
  • Understands the confidential nature of organizational information and maintains confidences.
  • Demonstrates financial prudence with funding resources.
PHYSICAL REQUIREMENTS

Employee must be able to:· Hear and speak to use telephones and communicate with people inside and outside the office.· Physical ability to move around the office area and in the community throughout each workday.· Visual ability to review detailed reports and information, and to use the computer for keyboarding.· Bend at the waist or crouch for filing charts in the filing cabinets.· Physical ability to do some lifting. TOOLS AND EQUIPMENT USED

The employee must be able to:· Use office equipment including postage machine, fax machine, computer, telephone system, copier, etc.· Have a valid driver’s license for the state of Minnesota and have an operating vehicle for transportation purposes. WORK ENVIRONMENT

  • Duties may be performed indoors in a school office, outdoors at the school, in a variety of community setting and client’s homes.
