Synopsis:

Under the general direction of the Site Medical Director, the Family Nurse Practitioner will be responsible for treating patients of all ages, regardless of gender, with a broad range of medical concerns, including routine health examinations and preventive screening, as well as acute and chronic medical conditions. S/he will also assist in supervising the services given by ancillary staff in order to ensure high quality care in a team based model of health care delivery.


Qualifications:

  • Masters Degree in Nursing
  • Nurse Practitioner Board Certification, Specialty of Family Nurse Practitioner
  • Current licensure to practice professional nursing in the District of Columbia or Maryland, as appropriate
  • Two years prior clinical work experience as a nurse practitioner, preferred
  • Two years prior work in a Latino community, preferred
  • Women’s health and HIV experience, highly desirable
  • Bilingual, Spanish-English
  • MS office or equivalent computer experience; experience with use of electronic health records


Duties & Responsibilities:

Clinical:

  • Provides primary medical care for a dedicated panel of patients, including diagnosis and treatment of acute and chronic illness according to evidence-based standards
  • Obtains clinical histories, conducts appropriate physical examinations and assessments, orders diagnostic testing and formulates treatment plans
  • Conducts routine health examinations and evaluations and makes recommendations for preventive health maintenance and screening tests as appropriate
  • Documents appropriate clinical information in the electronic health record in a timely manner, completing documentation of clinical encounters and locking progress notes within 48 hours
  • Reviews documents including outside records and consultant reports and summarizes information in the medical record
  • Determines need for additional services including but not limited nursing, social services, health education, and referral for specialty care
  • Participates in regular multidisciplinary clinical team meetings and assists in the coordination of care in a team-based setting
  • Orders and is responsible for the dispensing and administration of certain medications on-site
  • Delivers emergency care during clinic hours and is available to La Clínica del Pueblo patients for urgent clinical advice after hours, on a rotating basis
  • Refers patients to hospitals for emergency care and admission when needed, and communicates with hospital staff as appropriate
  • As directed by supervisor, supports the provision of team-based care of populations of patients, as appropriate to the role of a midlevel provider


Teaching & Training


  • Supervises health professional students and graduates as needed
  • Participates in continuing education and in-service programs including conferences relating to patient care
  • Participates in training to enhance ability to care for specific populations and conditions, as directed by supervisor


Administrative


  • Works closely with the site Medical Director, physicians and other midlevel practitioners, nurses, medical assistants and other La Clínica del Pueblo staff, patients, and other healthcare agencies and providers as appropriate
  • Assists the medical leadership in the development of clinical protocols as directed by supervisor
  • Participates in clinical quality improvement and risk management activities in collaboration with the clinical team
  • Shares after-hours phone coverage for patient emergencies
  • Performs other medically related services as required.
  • Is available to work at either of La Clínica del Pueblo’s clinical sites, as determined by schedule and as requested by supervisor, subject to state licensing requirements and reciprocity
  • Meets monthly with clinical supervisor to report and review clinical performance, documentation, quality of care measures, patient feedback and other topics as determined by supervisor
  • Represents the organization to the public, official organizations and other groups by attending conferences, meetings, etc., as designated by supervisor and as related to work duties.
  • Abides by La Clínica del Pueblo’s policies, procedures and requirements for clinical staff, including but not limited to those pertaining to clinical documentation, billing and coding, HIPAA and protected health information, and communication with LCDP staff and patients.
  • Supervises assigned volunteer staff.
  • Performs other activities as identified by supervisor.
