A growing accredited Maryland based mental health clinic is seeking a family nurse practitioner to provide direct somatic focused treatment services to individuals with co-occurring and chronic health disorders (health evaluations, consultations, medication management, referrals to specialized health treatments).

Service model for the caseload of 300 clients varies based on individual needs of client but typically includes initial evaluation, 1 monthly visit for somatic evaluations / med management, 1 weekly visit for substance use disorder med management.

All clients will be serviced from contractor's office at the same location. Some sessions are rendered via telehealth and some in person.

This a fee for service position. Pay is biweekly and is approximately $50-$75 per hour based on service provided. Services must be rendered fully onsite.

Reports to:

Medical Director (clinically) / Program Director (administratively)

Responsibilities include:

• Consultation with the behavioral health home staff as needed, reviewing or completing initial health home admission assessments as scheduled
• Consultation with the substance use disorder staff as needed
• Complete (or review) all treatment plans, admission assessments, continuity of care reviews, referrals to higher levels of care and diagnostic evaluations as scheduled
• Refer clients to other levels of care as needed
• Manage clients somatic medications as needed
• Schedule all sessions in advance in the ICANotes software
• Report to the main office weekly for paid administration time
• Document all services rendered in ICANotes electronic records management
• Check email and respond to call messages hourly while on duty
• participate in on-call after hour schedule as needed

Qualifications:

• Must be family nurse practitioner with privileges to practice in Maryland
• Willingness to be credentialed with insurance payors as a rendering provider under the facility
• Not barred from servicing medicaid / medicare patients
• Ability to efficiently use email, computer technology and electronic medical records software
• Delegating nurse certificate (or willingness to obtain one with Company's assistance)

Typical schedule:

• We are flexible you may set your own schedule / availability but prefer a set schedule at least a month in advance for treatment scheduling purposes
• Clinic is open Monday - Friday from 8:00 AM EST to 5:00 PM EST and after hours for crisis
• Must provide at least 40 hours per week of availability (break times are non-paid)

Compensation:

• Compensation is on a fee-for-service basis per documented treatment/service
• Ranges from $50 - $75 per hour for servicing a target of at least 2 clients per hour

Specific services:

• Complete medical (somatic) evaluation of all clients initially and monthly as needed 99203-99215
• Assist administration with determining best fit for new clients' residential settings
