We are looking for energetic, compassionate and professional people to work with program participants in their homes and in their communities. Self Hired Community Habilitation Assistants provide a wide variety of person-centered supports and services to people with intellectual and developmental disabilities helping them to live the life of their choice. Self Hired Staff will empower the people they work with by modeling, teaching and advocating for them to achieve their highest level of independent living, enable their fullest participation in society, ensure their civil and human rights are protected and promote their ability to be productive, contributing members of their community. Self-Hired Respite Assistants will provide support and supervision to a person with a developmental disability who resides in his/her family home, helping him/her to live out the life of his/her choice and providing relief to the family member/caregiver.

Self-Hired positions offer a variety of schedules and total hours. Program participants are located throughout WNY and the Southern Tier.

POSITION RESPONSIBILITIES

Maintain their first allegiance to the people they support by demonstrating respect and supporting the person in daily problem solving skills.

Provide assistance to people served one on one who are navigating their community, providing transportation when necessary to people served to social events and outings in their own personal vehicles.

Help individuals they support to develop and maintain positive interpersonal relationships with their peers, with their families and with those in their community.

Complete service documentation in an electronic medical record (EMR) system in accordance with all applicable state and federal rules and regulations.

Assist in the creation of the Self Directed Community Habilitation Plan, cooperatively working as part of the Circle of Support while assisting the person served to achieve their valued outcomes.

EDUCATION AND EXPERIENCE

• · Must hold a High School Diploma/GED

SKILLS, KNOWLEDGE AND ABILITIES

• · Read and write case notes and be able to follow written directions.
• · Manage multiple assigned tasks at one time and meet assigned deadlines.
• · Have basic computer knowledge and must be comfortable using web-based case note, time clock and employee portal systems.

VERIFICATIONS AND SCREENINGS:

• · Must successfully complete all pre-employment screenings including county clearance of criminal convictions, drug screen, and all New York State OCFS, Department of Motor Vehicles, Justice Center and OPWDD background checks prior to the commencement of employment.
• · Some positions will require candidate to hold and maintain a valid NYS Driver’s License; have access to a personal vehicle to use for work purposes and hold insurance on that vehicle.

PHYSICAL REQUIREMENTS:

• · Must be able to lift up to twenty (20) pounds.
• · Must be able to stay awake and alert while on duty.
• · Other physical requirements of the position may include: seeing, hearing, walking, sitting, standing, driving, stooping, bending, carrying, stretching, and climbing stairs.

WORKING CONDITIONS:

• · This position will work within locations in the community and in the homes of people receiving services. Work locations will vary as needs of person receiving services change.
• · Hours worked will vary based on needs of the person receiving services.

Job Type: Part-time

Pay: $16.00 - $20.00 per hour

Experience:

• Caregiving: 1 year (Required)
• Caregivers: 1 year (Required)

License/Certification:

• First Aid Certification (Preferred)
