Alliance Health at Braintree is a 101-bed skilled nursing facility. We are looking for a Director of Case Management – (RN/LPN) to join our management team!
$5000 Sign On Bonus!
The Case Manager will work with new admissions, assessing their conditions and needs to develop, evaluate, and update personalized care plans as needed. Also, providing education to the patients and their families about the proper way to follow the care plan and communicates with the ACO/HMOs as defined by contracts. The Director of Case Management also acts as the facility's MMQ Nurse. This position reports directly to the Executive Director.
A Licensed Nurse in MA with a minimum of (3) years of experience in a nursing management function is preferred. Must exhibit exceptional leadership, communication, management, and organizational skills. Being able to-multi-task is also a critical component to this position.

Pay: $1.00 - $2.00 per year
