Day/Vocational Program Case Manager Job at Keystone Community Resources

Job Description
Case Managers are responsible for the overall coordination of care, along with some responsibility for the overall and long term elements of the program. This is a float position, which means it will require travel to various Day and Vocational programs in the Scranton area. Additional duties include:
- Develops and Facilitates Individual Service Plans with client, family, funding agency representative, and members of treatment team;
- Responsible for the overall maintenance and quality of the clients’ file.
- Serves as a primary contact for families, agencies, and other professionals involved in clients’ care;
- Attends professional meetings as needed to represent the client and Keystone Community Resources;
- Provides necessary coverage in programs, in emergency situations, to ensure safety and welfare of clients;
- Participates in required in-service training for continuing professional development;
Qualifications:
- Bachelor’s degree in Human Services or a related human services field from an accredited college/ university and two years of work experience working directly with persons with disabilities OR Associate’s degree from an accredited college/university and four years of work experience working directly with persons with disabilities
- Must be at least 19 years of age
- Valid driver’s license with an acceptable driving record.
