Day Program Classroom Coach (GSBDH) - 35 Hrs/wk

Oversee a program classroom with approximately 13 adults with developmental disabilities: working together with three (3) co-workers!

Shift: Monday – Friday, 8:30A – 4:30P

Qualifications:

• Associate’s Degree
• Clean and valid NYS Driver’s License required
• HHA experience preferred

Responsibilities include, but not limited to:

• Plan, implement and evaluate activities that promote greater independence, inclusion, individuality and productivity for consumers.
• Maintain and log complete appropriate documentation for these activities.
• Assist consumer with all ADL needs including, but not limited to feeding, transferring, lifting, toileting, dressing (coats), etc. as per consumer’s protocols.
• Accompany and drive consumers into the community for programs.
• Document activities in the Individual Program Plan.
• Provide status reports to supervisor and peers.
• Attend staff meetings and participate in in-service training.
• Respond to parents/correspondents/advocates regarding consumers.
• Provide counseling, feedback and performance evaluations to Assistant Community Coaches.
• Cooperate with all clinical personnel to effectively deliver required services to program participants.
• Report any sickness, injury or seizure to medical personnel expeditiously.
• Complete department specific New Employee Orientation for period of one week to orient staff to various volunteer sites and daily routine of program.
• Write monthly reports.
• Ensure needs of consumers are being met including but not limited to their safety and well-being.
• Report situations/issues involving consumers to proper authorities.

The Cerebral Palsy Association of Nassau County is a nonprofit, multi-faceted health agency serving over 1,500 children and adults with cerebral palsy, developmental and other disabilities.

Day Habilitation is a year-round program serving individuals ages 21 and older who are developmentally disabled. Day Hab is a community outreach program where trained and qualified individuals with developmental disabilities provide local non- profit businesses with community service. It is a comprehensive program which focuses on community integration and volunteerism.
