Job Title: Day Habilitation Team Lead

Brief Job Description/Summary: Provides direct support and training services to individuals with intellectual, behavioral, and/or physical disabilities

List of Job Duties:

  • Participate in the development and implementation of person-centered planning and any other team meetings on behalf of persons served.
  • Instruct, supervise, and assist persons served in the development of social, leisure, recreational, communication, safety and other skills in accordance with their person-centered plan, and the scope of services as defined by Iowa Administrative Code.
  • Provide personalized and loving care, training and support based on the needs of the individuals and their written person-centered plan.
  • Assist persons served in the development of social, leisure, recreational, communication, safety and other skills
  • Plan, prepare and lead social and educational activities
  • Ensure environments are comfortable, productive, and stimulating.
  • Ability and willingness to assist with personal hygiene needs, and assisting with feeding
  • Plan, prepare, and engage in a variety of communityy-based activities (a.k.a community integration)
  • Utilize agency and/or public transportation, as needed
  • Provide Medication Management and health related services.
  • Provide thorough written documentation of services provided that include details of staff interventions, and how the consumer responded as outlined by funding source regulations.
  • Develop and maintain positive relationships with families, caregivers, referral sources, funding sources and other providers.
  • Communicate with family members and other care providers as directed (i.e., communication logs, etc.)
  • Maintain records of persons receiving services according to agency policies and procedures and regulatory requirements.
  • Serve lunch, take attendance, and supervise entrance/exit as needed.
  • Report incidents and other safety issues to supervisor.
  • Additional duties as assigned.

Required Experience and/or Knowledge, Skills and Abilities: If you have the ability to support and assist persons with disabilities, the ability to follow instructions and communicate effectively, and the ability to relate to persons served and their families, supportive leadership and on-the-job training will help you be successful in this job.
