Position Summary

This Position involves: training caregivers, hiring, onboarding, filling in for shifts, in call responsibilities

Reporting to the Branch Manager the Care Coordinator is responsible for many aspects of the Paw Paw and south east market.Including: Care Management ,Recruiting, Employee Development.

Responsibilities

• Using Clear Care / Well Sky to manage the Schedule
• Work with other team members to coordinate various aspects of a client’s care.
• Evaluate clients to determine appropriate service mix, including activities, personal care needs and need for outside services. Coordinate and liaise with outside service providers as needed. Evaluate and document safety issues and concerns and plan for addressing.
• Open new cases and complete service agreements and related forms with clients/families.
• Recruit, Hire and engage caregivers on your team
• Create a consistent schedule for clients and always fill shifts with call ins.
• Fill in for shifts if no other help is available- including over nights and weekends.
• Train caregivers

Position Requirements

• Extensive Caregiving experience including: Hoyer, Personal Care, Hospice and memory care
• Experience hiring and recruiting
• Comfortable with computers
• Well Sky Clear Care experience a plus
• Available with a normal 8a to 5p schedule with willingness to fill in when needed beyond those hours.
• Excellent verbal and written communication skills.
• Organizational skills, accuracy and attention to detail.
• Ability to work independently with minimal supervision.
