The REACH program is a crisis prevention, intervention and stabilization system to support individuals with intellectual and/or developmental disabilities, as well as a mental health condition or challenging behavior that is negatively affecting their quality of life. REACH provides consultation, prevention, development of crisis education & prevention plans, community based mobile support and crisis therapeutic home services within the counties of Arlington, Alexandria, Fairfax, Prince William, and Loudon. Position can be partially remote with some flexibility to the hours!!
Essential Job Functions:

  • Assists individuals with daily activities, personal care, behavioral health needs, and health, wellness and leisure activities in the home and community, and provides support at the REACH Crisis Therapeutic Home (CTH) when there are no individuals requiring assistance in the community
  • Establish rapport with guest/individuals with intellectual disabilities and challenging behaviors, while maintaining confidentiality and professional boundaries
  • Monitor individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation supports in accordance with crisis guidelines and person’s plan
  • Participates in development and implementation of persons’ Crisis Education and Prevention Plan
  • Plan, structure and prompt social and leisure time activities, that build skills for behavior management, stress management, communication, adaptive living and daily living.
  • Maintain timely documentation, progress notes, and other data, including incident/accident reports and discharge plans

Requirements:

  • Credentials as a Qualified Mental Health Professional (QMHP) and/or Qualified Developmental Disabilities Professional (QIDP) or able to be credentialed
  • Minimum of a bachelor’s degree in human services or related field received from an approved, accredited institution and no less than one year of fulltime relevant experience required
  • One year experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs required
  • Knowledge of the MH and I/DD systems of care
  • Knowledge and ability to utilize computer programs including but not limited to Microsoft Office Suite, Web-based data entry, etc.
  • Must demonstrate capacity to effectively communicate verbally and in written form
  • Valid, unrestricted, state-appropriate driver’s license and current personal vehicle insurance for required local travel
  • Proof of COVID-19 vaccination required
  • Must live in or near Arlington, Alexandria, Fairfax, Prince William, or Loudon counties
