SAGE is looking for team members living in the Conejo Valley area who are caring, patient, creative, empowering, and would like to share their gifts, skills, and talents with others! We are hiring for the Conejo Valley area (Newbury Park, Thousand Oaks, Simi Valley, and Moorpark). *Positions are only eligible for those residing in the Conejo Valley area*

You will need to have a valid driver’s license, current automobile insurance, access to a vehicle to transport individuals, provide a DMV report, and clear a department of justice (DOJ)/FBI background check/drug screening.

Current Opportunities:

1.) Conejo Coach (Conejo Valley Residents Only: $18.00/Hour): If you live in the Conejo Valley area and thrive in helping other, this is the right job for you! Supporting individuals with developmental/intellectual disabilities in their homes/community with daily living skills. As a SAGE team member, you would expect to support/train individuals in many aspects of their lives to promote health, wellness, empowerment, and growth such as social skills, vocational skills, meaningful activities, soft skills, grocery shopping, transportation, medical appointments/medication reminders, being a valued member of their community, meal planning/cooking, cleaning/laundry, personal care/hygiene (may include bathing, toileting, changing, dressing, oral hygiene), and behavioral support (may include but is not limited to self-injurious behaviors, property destruction, verbal aggression, and/or physical aggression), minor lifting/transferring with adaptive equipment, providing and/or training on transportation, behavioral support

*Conejo Coaches support multiple individuals throughout the Conejo Valley area. Conejo Coaches support individuals through Supported Living Services, Independent Living Services, In-Home Respite Services, and Day Program Services.

Available Schedules: Daily 8:00am-5:00pm, 8:00am-8:00pm, 12:00pm-10:00pm, 2:00pm-10:00pm, 10:00pm-8:00am (asleep overnights). *Part-Time/Full-Time*

2.) ILS Conejo Coordinator (Conejo Valley Residents Only: $24.00/Hour daytime hours): If you live in the Conejo Valley area, are highly organized, have amazing customer services skills, and thrive in helping others, this is the right job for you! Oversees/trains/supports team and assigned caseload of individuals (all ages), enforces policies and procedures, manages documentation/data collection/systems/communication, schedules/facilitates/attends trainings, meetings, appointments, completes all reporting requirements, schedules assigned team, meets deadlines, develops/implements objectives/programs with individuals for skill building/maintenance which may include but is not limited to accessing resources, laundry, medication, medical appointments, money management, grocery shopping, meal planning/preparation/cooking, safety/emergency skills, health & wellness, community connecting/activities, cleaning, personal hygiene/grooming (may include bathing, toileting, changing, dressing, oral hygiene), minor lifting/transferring with adaptive equipment, providing and/or training on transportation, behavioral support (may include but is not limited to self-injurious behaviors, property destruction, verbal aggression, and/or physical aggression).

Available Schedules: Maintains a flexible weekly schedule either Sunday-Thursday or Tuesday-Saturday. *Typical work hours will be up to 40 hours a week, 5 days per week, between 8:00am-10:00pm, depending on services and support needs for everyone on assigned caseload.
