AHRC New York City is seeking a Community Support Professional (CSP) our Day Services program supporting adults with intellectual and other developmental disabilities.

Monday thru Friday 8:30a to 4:30p

Under the direction of the Program Supervisor, the Community Support Professional (CSP) will:

Support individuals with disabilities in leading self-directed lives.

CSP will collaborate with individuals and others to plan and implement supports that build upon individual strengths, choices and capabilities.

CSP will also develop and expand meaningful community opportunities.

CSP will work with the supervisor on developing and implementing an effective plan, and will ensure compliance with OPWDD regulations, agency policies, and agency values.

General Responsibilities:

Supports individual with disabilities in becoming valued members of their communities. Supports behavior that enhances inclusion in their communities.
Provide direct training and support in individuals’ home and/or community.

Empowers individual and their family by providing the support and information necessary to make informed choices and decisions.

Helps build the self–esteem and assertiveness necessary to lead a self-determining life.

Use formal and informal methods to learn about the needs, interests and desires of individual.

Assists individual in identifying the formal and informal supports available in his/her community, family and social network and works with the individual to develop and sustain relationships with friends, family and community members.

Assists the individual in managing every day aspects of life by utilizing habilitative strategies and providing supports in a manner that builds on individual strengths and capabilities and maximizes choices.

Pursues opportunities for professional growth and shares knowledge and resources with others. Assists the individual to overcome challenges through effective advocacy and the facilitation of self-advocacy.

Qualifications

To perform this job successfully, an individual must be able to perform essential responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Knowledge of Home and Community Based Waiver, and the ability to keep up to date pertaining to such knowledge, and interpret same.

Understanding of Person Centered Planning.

Reasonable flexibility in work hours and ability to respond to the scheduling needs of the individual. May include early morning, evening and/or weekend hours.

Ability to establish and maintain a working relationship with program participants, families, staff and community agencies.

Ability to communicate both orally and in written form in English. Ability to organize work with self and others.

Education Requirements:

AA/Bachelor’s degree and experience with individuals with disabilities preferred.
