ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

  • With PCP guidance and direction, conducts personalized, in-home health assessments with senior patients. This includes documenting vital signs and measuring weight and relevant physical attributes as well as, identifying cognitive, functional, medication and social challenges impacting patients health.
  • Communicates health updates with the patient’s PCP or the on-call PCP/provider should the patient present immediate health risks. Schedules same-day appointments when necessary. Ensures a physician-directed medical intervention as needed.
  • Prepares patients for virtual examinations; performs routine screenings including EKG and neuropathy. Assists physician with exams and explains treatment procedures and physicians' instructions to patient.
  • Prepares and administers medications including IM and IV, changes dressings, applies bandages, removes sutures and other first aid procedures. Performs CPR when necessary.
  • Performs non-critical medical and therapeutic procedures based on medical instructions. Collects phlebotomy and other lab specimens; performs basic Point of Care lab tests.
  • Interacts with EMS and emergency department personnel as necessary and with expediency, to coordinate emergency care for any presentation warranting an immediate trip to the hospital.
  • Creates and records detailed File of Life documents for each patient, noting all medications, allergies and medical conditions. Documents contact information for each family member or friend serving as emergency contact.
  • Documents and reports findings/actions to ensure patients’ visit are accurately recorded and received by the PCP as soon as administratively possible. Provides recommendations to address/reduce health risk.
  • Maintains home visit supplies, equipment and stocks. Sterilizes instruments, practices OSHA safety standards, practices accurate, legal and ethical documentation at all times.
  • As defined by Florida Statutes and pursuant to medical protocols, policies and procedures set by the PCP and/or Medical Director, administers appropriate patient care at the EMT/Paramedic skill level in emergency medical situations.
  • Reports cases of neglect or abuse to Adult/Child protective services. Contacts police when appropriate.
  • Educates seniors on aging and disability resources and other applicable social services. Coordinates needed mental health interventions, leveraging community resources whenever possible. Coordinates fire rescue units, police and other community agencies making home visits.
  • Performs other duties as assigned and modified at manager’s discretion.

In-center responsibilities, covering for medical assistants as necessary:

  • Maintains proper documentation in the EHR: indexing records, updating profiles and scheduling visits as appropriate.
  • Supports virtual patient visits by delivering visit links, activating virtual visits and ensuring the patient and doctor are able to access the visit.
  • Monitors and processes patient requests from phone, patient portal and mobile app. Returns outreach and routes messages to other team members as appropriate. Contacts patients to obtain and relay pertinent information for/from the physician.
  • Upon physician approval and authorization, calls in prescriptions to the pharmacy. If authorized, utilizes the ChenMed Rx system to support physician medication dispensing.
  • Conducts check-ins and checkouts, monitors inventory, processes medical records requests and supports specialty referrals.

  • Ability and willingness to travel.
  • Spoken and written fluency in English
  • Willingness and ability to be on-call evenings and weekends.

EDUCATION AND EXPERIENCE CRITERIA:

  • High School diploma or equivalent required
  • A minimum of 1 year of work experience as a Paramedic or EMT. Comparable relevant experience in public health or other health-related field may be considered in lieu of paramedic experience
  • Current unrestricted Paramedic state license required
  • Basic Life Support (BLS) certification required.
  • Active driver’s license from authorized state agency
  • Access to reliable, fully insured vehicle
  • Must live and/or work in or near a community where ChenMed operates
  • Experience working with geriatric patients is a plus

Job Type: Full-time

Pay: $25.00 - $29.00 per hour
