Community Health Workers (CHW) will amplify COVID-19 messaging to priority communities across LA County (in-field and out-field outreach). Community Health Workers (CHW) will work directly with the Chief Programs Officer and community health workers to perform a wide range of complex and confidential responsibilities around COVID outreach, education, and Contact Tracing. Supervisor oversees and works with Community Health Workers to meet initiative goals. Supervisor also ensures each staff is certified and receives appropriate trainings on COVID, Contact tracing, and Trauma and Resilience.

QUALIFICATIONS

  • High School Diploma or College degree
  • Three years of more in medical services and/or coordination of care
  • Experience in case management
  • Experience in supervision
  • Ability to facilitate various deliverables at once
  • Arrange schedules and training needs
  • Planning and coordination of a program and its activities.
  • Ensuring implementation of policies and practices.
  • Track data, files, and documentation
  • Knowledge of reporting.
  • Ability to establish and maintain effective working relationships with associates, physicians, individuals, and the public.
  • Ability to exhibit a professional, positive attitude and work ethic
  • Excellent interpersonal skills required and ability to interact professionally with culturally diverse individuals during a time of crisis and distress
  • Ability to show empathy to distressed individuals
  • Excellent organizational and communication skills
  • Critical thinking and sound judgment required
  • Ability to handle confidential information with discretion and professionalism
RESPONSIBILITIES

  • Provide comprehensive education and information through street outreach, health fairs, school relationships, community events, virtual outreach, and other approved activities on COVID.
  • Enters and Supervises data collection via County Web-based platform-activities tracked in real-time after EVERY outreach – 10-12 Questions
  • Increase awareness of COVID 19, safety, and resources
  • Distribute reusable face masks, hand sanitizer, fliers
  • Contacts COVID-19 positive patients to establish the case history and contact list.
  • Communicates with community and contacts in a professional and empathetic manner. Records in log and/or ECW as necessary
  • Establishes the description and timeline of onset of symptoms and interviews patient to determine all possibly exposed individuals.
  • Obtains contact information for each of those individuals and follows the script to inform contacts about the importance of quarantine and/or testing
  • Explains information and recommendations regarding isolation and quarantine parameters, services available to patient, and monitors patient for ongoing needs such as a COVID test and access to a medical appointment at SJWCFC
  • Helps patient develop an appropriate plan which may include a test, medical appointment, and/or referral to another agency or professional.
  • Maintains detailed records of patients and assemble these into periodic reports.
  • Data entry and familiarity with computer software programs
  • Provides information and assistance regarding health insurance and benefits
  • Coordinates and facilitates the enrollment of patients to local, state and county programs
