Community Health Worker Job at Peoples Family of Corporations
About Us:
Too often, underserved communities are preoccupied with the challenges and activities of daily living at the expense of focusing on family health maintenance and wellness issues. In response to these and other community concerns, People’s Family of Corporations has established “community support” corporations that have proved successful beyond its well-known Betty Jean Kerr People’s Health Centers.
Responsibilities:
Assist with referral tracking and feedback
Assist with performance improvement and data management
Ensures all outgoing reports with client information are encrypted
Maintains and organize master client list, chronic disease patients that are noncompliant with appointments
Maintain/updates appointment report daily
Provide assistance with enabling services such as transportation, food, housing, etc.
Remind patients regarding keeping appointments, filling prescriptions, follow-up on self-management goals, etc.
Requirements:
High School Diploma Required
College degree in Health care or Business Management preferred
LPN/MA/CHW would be helpful
