Compensation:

  • $25/hour
  • Paid time off
  • Paid travel time for trips lasting more than 45 minutes.
  • Mileage reimbursement for trips beyond 20 miles.

Location: This position would be performed at Chicagoland outpatient medical clinics.

The Community Health Worker (CHW) would be responsible for providing outreach services for people with spina bifida and other central nervous system (CNS) congenital anomalies, such as hydrocephalus, Chiari malformation type 1, craniosynostosis, and syringomyelia.

No prior experience in health care is required. Training will be provided.

The CHW would be an employee of the Illinois Spina Bifida Association.

The CHW would provide services at one or two outpatient clinics per week.

  • One day would be staffing a clinic that is part of the Chicagoland Children’s Health Alliance, serving people with a range of CNS congenital anomalies. Outreach at these clinics would be directed by the University of Chicago Medicine Margaret Hackett Family Program in CNS Congenital Anomalies.
  • The other day would be staffing a clinic that serves children and adults living with spina bifida. Outreach at these clinics would be directed by the Illinois Spina Bifida Association.

The clinic schedule will vary week to week but is planned two months in advance. Clinics may last from 3-8 hours. Remaining hours are performed remotely, with a flexible work schedule. ISBA will provide the employee with a laptop computer and wifi hotspot.

Candidates will be evaluated in part on their ability to fulfill the roles and competencies of a Community Health Worker.

The Illinois Department of Public Health defines a community health worker as “a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. A community health worker also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy”.

For this position the Community Health Worker would be responsible for:

  • Engaging the relevant patients in an effective and discrete manner in an outpatient setting
  • Efficiently communicating with the patients about available programs and services
  • Identifying and documenting areas of patient needs and interests

Responsibilities

  • Conduct outreach to individuals and families at outpatient clinics in Chicagoland.
  • Develop rapport and earn trust with outpatient clinic staff and patients.
  • Provide timely and accurate record keeping of outreach activities.

Minimum Qualifications:

  • Full COVID vaccination and annual flu shots
  • Regular weekday availability
  • Driver’s license, auto liability insurance, and reliable vehicle

Required Abilities:

  • Proficiency with Microsoft Office and ability to learn new applications like Salesforce
  • Ability to work, prioritize, and problem-solve independently
  • Attention to detail and data entry proficiency

OPTIONAL Preferred Qualifications:

  • Sales or marketing experience
  • Spanish language proficiency
  • Health care experience
