The Community Health Specialist 2 will develop and conduct community health activities, provide social determinants of health resources, health information, advocacy, social support, and assistance in using the health care system to Primary Care clients and their families.

As a Community Health Specialist 2 you will:

  • Respond to direct referrals from providers and from the County Call Center (Covid-19 wraparound referrals) for/to based on chief complaints in Epic under the Person Centered Medical Home Umbrella.
  • Coordinate services as needed by clients in collaboration with members of the care team; for complex system navigation (insured or uninsured); and accompany clients to specialty appointments.
  • Home Visits and deliveries to client homes to support referrals, including Covid referrals.
  • Provide community and social support for services such as school enrollment, assistance connecting to external services like SNAP, WIC, DHC, Aging & Disability Services.
  • Develop and deliver activities designed to promote individual and community health.
  • Provides education that assists patients in identifying goals for behavior change such as Healthy Heart Ambassador, Walk with Ease, COVID support.
  • Health promotion and health education in the clinical setting including outreach and community engagement.
  • Supporting the resource, referral, and healthcare needs of the assigned teams (typically 2-4) in order to develop plans based on the specific population.
  • Assess strengths and needs of individuals, families and communities; recognize and identify social and medical risk factors.
  • Plan and share interactive health education classes and presentations for a variety of groups.
  • Develop and maintain effective working relationships with members of the care team to participate in regular team, group and individual case conferences.
  • Based on life/work experience, share community perspectives and needs with colleagues in the health and social service systems.
  • Maintain necessary written documentation of activities in Epic electronic health record.

Minimum Qualifications/Transferable Skills*:
Any combination of experience and training that would likely provide the required knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be:
  • Education/Training: Equivalent to completion of the twelfth grade AND
  • Experience: Two years of experience in community outreach services providing health information, advocacy, social support and assistance in using the health care system to groups and families; AND experience working with the community being served.
  • CPR certification is required.
Preferred Qualifications: CHW (Community Health Worker) Certification
COVID-19 Vaccination Requirement:
To protect the health of the community and employees, Multnomah County requires employees to be fully vaccinated against COVID-19 or have an approved medical or religious exception as a qualification of employment.
The rule applies to all employees, as allowable by law. Candidates who receive an offer of County employment must provide proof of vaccination upon hire or submit an exception request prior to their start date.
