Community Case Management Program Manager (Houlton) Job at Community Living Association
Job Summary
Supervise a team of Community Case Managers. Ensure staff required trainings are completed; and effective high quality casework and case management services. Develop and review client person centered plans; assess client progress and overall program effectiveness; problem solve and develop actions plans to address issues. Maintain compliance with DHHS and MaineCare policies, rules, regulations, and EIS documentation requirements. Maintain a partial client caseload as needed. Promote positive relations with parents/guardians, staff, agencies, and community. Full-time, 40 hour position. Excellent compensation and benefits commensurate with education and experience.
Requirements
Bachelor’s degree in Social Work, Human Services, or related degree, and experience working with people with intellectual and developmental disabilities. Supervision experience is preferred. Good computer (MS Word, Excel), administrative, organization, communication, and attention to detail skills required. Maine driver’s license. Must be willing to travel throughout Aroostook County as required for client visits and/or supervision of staff.
