II. Job Summary:


This purpose of this position is to provide leadership to the person-centered delivery of services and care for residents in Clare Housing’s Care Homes. The Community Care Homes Program Manager provides direct supervision to Lead Caregivers, the Weekend Care Home Manager, and full time Caregivers in the Care Homes.


III. Core Job Functions:


Duties and Responsibilities:

1. Supervise and provide support to 40+ full-time and/or part-time Caregivers in a residential care setting.

  • Recruit, hire, train, supervise and evaluate caregiving staff to ensure quality of care for residents.
  • Ensure all orientation and training requirements for Caregivers under the 245D licensure are met and documented.
  • Facilitate agency Education & Training Group meetings on a monthly basis.
  • Develop and deliver professional trainings to staff.
  • Organize and track training hours for over 40 staff persons per 245D requirements.


2. Ensure excellent quality of person-centered care for the residents living with HIV/AIDS in Care Homes.

  • Assess regularly the status of each resident in the home.
  • Lead staff in the use of Positive Support Strategies when supporting clients.
  • Provide leadership and direction to caregiving staff to meet client needs in the least restrictive manner possible.
  • Communicate regularly with internal and external professionals to deliver and coordinate care.
  • Meet weekly with Lead Caregivers, Social Worker and consulting professionals to review residents.
  • Ensure staff is meeting service delivery requirements in Coordinated Service and Support Plans.
  • Accompany and/or transport residents to appointments as appropriate.


3. Assess residents and facilitate the new resident selection process.

  • Interview and assess client for program eligibility.
  • Maintain 95%+ occupancy in the care homes.
  • Responsible for completing resident placement and licensing paperwork per 245D.
  • Assist with developing and updating Resident Handbook for Care Homes.


4. Communicate with outside professionals to coordinate resident care and services.

  • Communicate regularly with CADI and HIV case managers.
  • Troubleshoot external service delivery such as transportation, mental health services, etc.
  • Communicate with clinic nurses, physicians and pharmacies on an as needed basis.


5. Manage the community care home program budget and fiscal responsibilities.

  • Manage the caregiving schedule in an efficient manner.
  • Create effective plan for procurement of household items/groceries.
  • Oversee the payroll process for Care Home staff.


IV. Position Requirements:


Education and Experience:

  • Bachelor’s degree or equivalent combination of education and/or related work experience.
  • Program management and program budget management experience.
  • Three years of supervisory experience highly preferred.
  • One or more year of HIV/AIDS managerial experience with Clare Housing is a plus.


Knowledge, Skills and Abilities:

  • Ability to attract, retain and motivate top quality Caregivers.
  • Advanced knowledge of HIV/AIDS disease and medications.
  • Ability to read, write and speak English sufficiently in order to communicate pertinent medical and social services to residents and to communicate with management and staff.
  • Have a sensitivity and aptitude for working with individuals who were formally homeless and persons living with HIV/AIDS, mental illness and chemical health problems.
  • Ability to work respectfully and communicate effectively with multi-cultural, diverse populations in what can be at times stressful and emotional situations.
  • Be able to lead a team of staff persons leading them to deliver person-centered services and Positive Support Strategies.
  • Excellent management and staff leadership skills.
  • Strong knowledge of emergency procedures and be able to stay calm in stressful situations.
  • Ability to maintain professional boundaries and practice self-care in a position that is confronted often with physical illness, mental illness and chemical abuse issues.
  • Ability to work independently and collaboratively, exercise good judgment and pay close attention to detail.
  • Excellent interpersonal communication skills; verbal and written.
  • Strong organizational skills and the ability to multi-task.
  • Ability to take and pass the Medications Administration Training.
  • Ability to maintain confidentiality in both client and employee information and privacy of residents.
  • Ability to work within an interdisciplinary team.
  • Must be able to relate professionally with residents, staff, physicians, and case managers.
  • Must have the emotional stability needed to cope with the mental and emotional stress of the position.
  • Access to reliable vehicle and ability to maintain good driving record and insurance levels, as directed by the agency, to fulfill job duties that require travel into the community.

Licenses and/or Certifications:

  • CPR/First Aid Certification required; training provided to obtain certification.
  • Driver’s License, State ID or Passport


V. Working Conditions and Environment:


This position is co-located at three Care Homes in South Minneapolis for adults living with HIV/AIDS and other disabilities. Many of the residents are formerly homeless individuals. The buildings are accessible and smoke free. This position works during normal business hours which may include evening and weekend hours.


This position requires extensive physical activity, as Care Home staff provide direct, physical care for four individuals in each home. Caregiver tasks include doing laundry, housekeeping, cooking, lifting/transferring residents and other physical tasks. The Care Home Program Manager position requires the employee to be committed to delivering person-centered services and to understand Positive Support Strategies. This position requires the manual dexterity to use various types of medical equipment, as well as the cognitive ability to perform more detailed tasks such as medication administration, charting, filing and copying.
