Platte County Legacy Home in Wheatland Wyoming is seeking a full time Certified Nurses Assistant for our Assisted Living.

Platte County Legacy Home strives to provide a continuum of excellent care and services to the aging in a spirit of love, respect and compassion.

Job Summary:

Job Qualifications:

1. Have successfully completed a formal certification training program and passed the Certified Nurse Assistance Exam and have a current licensure with the Wyoming State Board of Nursing

2. Be at least eighteen (18) years of age.

3. Have experience in a health care facility.

4. Possess the ability to read, write and carry out directions.

5. Possess the maturity to deal effectively with the demands of the job.

6. Have demonstrated the ability to work independently and make appropriate judgments

7. Have demonstrated dependability, tact, and ability to follow orders.

8. Possess and maintain good physical and mental health, including current TB testing

9. Have U.S. citizenship or evidence of valid Alien Work Permit.

10. Must stand current with Relias education

Resident Service Function: The following services should be provided in accordance with the resident’s Service Plan. These services should be provided in a manner that encourages and maintains the resident’s independence to the greatest extent possible.

Provide assistance as needed with:

· Dressing and undressing

· Bathing

· Toileting

· Personal hygiene, shaving, dental hygiene, nail care, hair care, foot care, etc.

· Mobility devices (cane, walker, wheelchair, crutches) including monitoring of safe transfer and ambulation techniques.

· Care of personal adaptive devices such as eyeglasses, contact lenses, hearing aids.

· Dining

· Bed making and bed changes

· Trash disposal

· Laundry

· Activities (independent and group)

· Transportation and business affairs

· Appointments away from facility (coordinating with RN Director and Transportation Driver).

· Services by providers from outside agencies, provided within the facility (home health, etc.)

Housekeeping Functions

· Performs daily housekeeping tasks as assigned

· Clean floors, to include sweeping, dusting, damp/wet mopping, disinfecting, etc. (Assure that appropriate caution/safety signs are properly set up prior to performing such duties.)

· Clean mirrors as needed

· Clean resident’s bathrooms per schedule

· Keep staff bathroom clean

· Clean laundry area as needed

· Discard waste/trash into proper containers and reline trash receptacle with plastic liner. Discarding infectious wastes into appropriate containers.

· Ensure that resident’s rooms have needed supplies (toilet tissue, facial tissue, disposable hand towels, etc.)

· Maintain supplies in staff restroom, tub room and laundry room

· Clean up spills and stains as necessary

· Maintain communication with Housekeeping Supervisor as to needed supplies and equipment

· Notify Housekeeping Supervisor when major housekeeping functions are needed (Cleaning carpets, cleaning Four Season’s room, Cleaning Dining room, cleaning windows, etc.)

Activity Functions

· Assist Activity Department in development of activities, independent and group, that conform with resident’s desires, preferences and per personal service plan

· Participate, as directed, in providing and assisting with activities for the residents

· Notify Activity Director of requests by the resident for activities or activity supplies

· Other Activity functions

Dining Functions

· Assist Dietary staff in serving meals

· Assist residents with meals as needed (cutting meats, opening packets, pouring drinks, etc.)

· Assists in clearing tables as needed, gathering serving ware and delivering to dishwashing area for cleaning

· Maintaining dining room in a clean and sanitary manner

· Maintaining adequate supply of snacks and other food items in the dining room/kitchen area as directed.

· Notify Dietary Department of special requests, guests who request meals, etc., as soon as requests are made to ensure requests are honored

· Other dining functions as directed

Social Service Functions

· Monitor residents for signs of depression, anxiety, withdrawal, or loss of cognitive ability. Notify Social Services Director of concerns.

· Encourage residents to maintain social contact (coming to activities, meals, church services, etc.)

Laundry Functions

· Provide clean linens in each resident’s room daily

· Gather dirty linens and personal laundry on scheduled laundry day to be washed by Night staff.

· Assist residents as needed, to launder personal items in public laundry

Environmental Service Functions

· Ensure that all equipment, fixtures and furniture is clean and in good repair

· Monitor resident rooms and common areas, and identify potential hazards

· Report maintenance issues promptly to Maintenance Director

· Utilizes equipment according to manufacturer’s recommendations and Platte County Legacy Home policy.

Documentation and Reporting Functions

· Ensures care is provided on basis of physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements

· Maintain a working knowledge of each Resident’s Service Plan, including review for changes in condition or needs

· Document narrative notes, at least monthly, describing resident’s condition

· Document vital signs weekly and weights at least monthly or as ordered

· Documents assessment, planning, implementation and evaluation of care. Including monthly summary of resident status as assigned. Documentation is timely, accurate and complete

· Evaluates the resident’s and family’s response to care.

· Implements the service plan of care through direct and indirect resident care

· Follows and documents a service plan for each resident, utilizing assessment data, resident, and family with care team input.

· Document narrative notes, vital signs, and other information when resident has a change of heath condition, a fall, a change in behavior, or any other incident.

· Provide written and oral report of resident’s condition, or change in services provided to Assisted Living Director and to other staff as appropriate

Staff Development /Educational Requirements

· Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your license on a current status.

· Keep CPR certification current

· Attend and participate in orientation programs as necessary

· Participate in an in-service training program prior to performing tasks that involve potential exposure to blood or body fluids.

· Assist in training and orientation of new Assisted Living staff

· Complete Relias training monthly and keep current on all training

Safety/ Sanitation/Infection control

· Follows standard precautions in the provision of patient care and implements other infection control measures as necessary

· Use personal protective equipment when performing procedures that involve exposure to blood or body fluids

· Report all hazardous conditions and equipment to Assisted Living Director or Platte County Legacy Administrator immediately. Reports/removes unsafe equipment following Platte County Legacy Home facility guidelines.

· Follow established safety precautions in the performance of all duties

· Wash hands before and after performing any service for the resident

· Follows PCMNH emergency and disaster preparedness plan.

Resident Rights

· Be informed. Have strong knowledge and practices of resident’s rights

· Maintain the confidentiality of all resident care information

· Ensure that residents interviews are conducted in private

· Report any complaints or grievances made by residents to the Assisted Living

Director or the Platte County Legacy Home Administrator

· Ensure that you treat all resident’s fairly and with kindness, dignity, and respect.

· Knock before entering the resident’s apartment

· Report incidents or suspected incidents of resident abuse to the Assisted Living Director or to the Platte County Legacy Home Administrator immediately

· Honor the resident’s refusal of treatments or medications request and document reason for refusal and report to the Assisted Living Director

Working Conditions

· Works in office area as well as throughout the nursing service area (i.e., drug rooms, nurses’ stations, resident rooms, etc.).

· Sits, stands, bends, lifts and moves intermittently during working hours.

· Is subject to frequent interruptions.

· Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.

· Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.

· Communicates with the medical staff, nursing personnel, and other departments providing resident care.

· Works beyond normal working hours and on weekends and holidays on a rotating basis with fellow staff.

· Is involved with promoting our facility in the community by attending community functions, programs and projects as appropriate.

· May be subject to falls, odors and personal incidents, etc. throughout work day.

· Is subject to exposure to infectious waste, diseases, etc., including TB and the AIDS and Hepatitis B viruses

· Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the resident’s needs.

· Follows established procedures governing the handling and disposal of infectious waste

Specific Requirements

· Must be able to read, write, speak, and understand the English language

· Must possess the ability to make decisions when circumstances warrant such action

· Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

· Must possess the ability and willingness to work harmoniously with professional and non-professional personnel

· Must have patience, tact, and a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.

· Must be able to relate information concerning a resident’s condition to their family and their physician

· Must not pose a direct threat to the health or safety of other individuals in the workplace

· Is prompt and efficient with minimal absences

Physical and Sensory Requirements

· Must be able to cope with the mental and emotional stress of the position

· Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met

· Must function independently, and have flexibility, personal integrity and the ability to work effectively with residents, personnel, and support agencies

· Must be in good general health and demonstrate emotional stability.

· Must be able to relate to and work with the disabled, elderly, emotionally upset, and at times hostile people within the facility.

· Must be able to lift, push, pull and move a minimum of 25 pounds. Must understand and use good body mechanics

· Must be able to assist in the evacuation of residents during emergency situations.

Competent with equipment use:

Electronic and manual blood pressure equipment, pulse oximetry, glucometer, temporal thermometer, portable suction, whirlpool tub, wheelchairs, nurse call system, O2 tank and regulators, Oxygen concentrators, trapeze, microwave, television, computers, printers, telephones, paper shredder and copiers. Whirlpool bath. Washer and dryer.

MINIMUM QUALIFICATIONS:

· Must possess a current valid Wyoming CNA license with a current BLS.

· Experience in the clinical area for which he or she is applying is desired.
