GENERAL SUMMARY
Collaborates with the Site Health Center Operations Director regarding all aspects of site management; collaborates with Senior Provider Leadership and Human Resources to recruit, retain, and develop clinical providers; improves the quality of care of patients at the site by working with site medical providers, QI and Erie provider leadership to establish evidence-based best practices for the site; leads the clinical team to insure cohesive care throughout the site; maintains referral relationships with the site’s hospital partners; maintains relationships with Residency Program Directors and surrounding hospitals regarding Resident Physician training when appropriate; gets to YES with Erie’s patients and employees to help provide service excellence.

PRINCIPAL DUTIES AND RESPONSIBILITIES

  • Collaborates with Site Health Center Operations Director to ensure successful clinical operations at the site.
  • Accountable for the quality of care, patient satisfaction and clinical productivity for the site.
  • Actively participates in selecting providers to work at the site. Orients, coaches/mentors, evaluates and disciplines clinical providers for the site.
  • Implements and achieves performance goals and objectives that are set by the Senior Medical Directors and the Chief Clinical Officer for both individual providers and at the site level
  • Communicates with providers regularly in both written and verbal communication regarding changes in workflows, new Erie initiatives and other important updates.
  • Implements Erie’s clinical peer review/evaluation process in conjunction with Chief Clinical Officer, Senior Medical Directors and Human Resources.
  • Coordinates scheduling of clinical providers for patient care hours for the site, considering providers’ inpatient responsibilities, after hours call and hours at other clinical sites. Participates as requested in the coordination and production of call schedules and schedules for hospital responsibilities.
  • Identifies staffing needs for clinical providers at the site and communicates these to the Senior Medical Directors. Works with other Site Medical Directors and the Senior Medical Directors to facilitate adequate clinician coverage at all sites when needed due to short term gaps in staffing (such as FMLA or clinician resignation).
  • Implements and maintains compliance with EFHC’s policies, procedures and guidelines for the care of patients at the site to assure care of the highest quality and maintain compliance with Joint Commission, HRSA, FTCA, Managed Care, PCMH, Meaningful Use, and other applicable standards.
  • Identifies risk management issues related to clinical care and communicates these to the Senior Medical Director who they report to.
  • Identifies quality improvement issues related to clinical care and collaborates with the Senior Medical Directors and/or Site Health Center Operations Director for the implementation of any inquiry or intervention as deemed necessary.
  • Participates in the preparation of site’s budget in conjunction with the Site Health Center Operations Director. Shares responsibility for its successful implementation.
  • Participates in strategic planning for EFHC as requested.
  • Collaborates with Residency Program Directors when appropriate to assure close and productive clinical, teaching, mentoring and research relationships that support EFHC’s and the residency program's goals.
  • Collaborates with Delivery Team Leader when appropriate to support reproductive health providers and provide feedback to them when needed
  • Attends Leadership Team Meetings, provider leader meetings, and joint SMD-HCO meetings.
  • Collaborates with the Site Health Center Operations Director to facilitate the monthly Site Meeting and Site Management Meeting
  • Builds the organization’s reputation through relationship development and maintenance with partner organizations and community leaders.
  • Initiates, supports, and/or participates in research and continuing education activities and is active in local, state and national issues affecting community medicine.
  • Interprets the philosophy of EFHC to staff and clients.
  • Performs clinical functions as described in the job description for physicians in their discipline.

QUALIFICATIONS

  • Physician, APRN or PA licensed to practice in Illinois required
  • Board Certified or Board Eligible in a primary care discipline
  • MD / DO in Family or Internal Medicine preferred to support Swedish Family and Internal Medicine residencies; not required
  • Two or more years of post-residency or APRN / PA clinical primary care experience required
  • Previous leadership experience preferred
  • Additional master's level degree, or equivalent experience: public health, management and administration, or related field preferred
  • Basic rescuer CPR certification
  • Bilingual fluency (English/Spanish) preferred (Will be required to assist or work with non-English/Spanish only speaking patients and/or guests.)
