Reunion Rehabilitation Hospital Phoenix is looking for an outgoing and experienced Clinical Liaison. This individual must be passionate about patient care and will be responsible for engaging and educating our community and referral sources. We are a 48-bed inpatient rehabilitation hospital, that opened in January 2022. The hospital is operated by Nobis Rehabilitation Partners and brings approximately 175 new jobs to the community across multiple therapy disciplines, rehab nursing, and hospital support team members.

The Clinical Liaison acts as a coordinator between internal and external referral sources, and the acute inpatient rehabilitation operations. The Clinical Liaison is responsible for establishing relationships with case coordinators, physicians, payors, and other referral sources. The Clinical Liaison will ensure appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Coordination and the Inpatient Rehabilitation Medical Director. The Clinical Liaison works closely with hospital Case Coordinators and the acute inpatient rehabilitation admissions team to ensure a responsive and smooth flowing referral and admission process.

ESSENTIAL FUNCTIONS:

  • Utilizes appropriate patient information sources as well as direct communications with all members of the patient’s care team to identify those patients who may need acute inpatient rehabilitation services prior to returning home. Reviews these patients daily to ensure timely transitions to inpatient rehabilitation as appropriate.
  • Assists and coordinates the intake and pre-admission screening process; accurately and timely documents evidence of the patient meeting the established acute rehabilitation admission criteria on the Pre-Admissions Screening form.. Provides timely information to the Acute Rehabilitation Medical Director and responds to any questions or requests for additional information necessary for appropriate decision-making.
  • Performs on-site and off-site review of patients in designated facilities to assist in identifying patients who could clinically benefit from acute inpatient rehabilitation services.
  • Performs on-site and off-site clinical evaluations to assist the Acute Rehabilitation Medical Director in determining appropriateness for admission.
  • Provides written materials designed for the Acute Inpatient Rehabilitation Unit to Case Coordinators to share with patients and family members. May meet face to face with patients and families once accepted into the program and transition is planned.
  • Communicates to patients and families the expectations and requirements for participation, in an acute inpatient rehabilitation program; offers the patient’s family a tour of the unit and assists in coordinating the tour as needed..
  • Gathers information related to the patient’s financial resources and benefits.
  • Demonstrates clinical proficiency and applies knowledge of acute inpatient rehabilitation admission criteria to identify appropriate patients who would benefit from the program.. Advises referral sources on acute inpatient rehabilitation criteria.
  • Creates target lists of contact and decision makers in assigned hospitals. Develops relationships, provides excellent customer service and educates referral sources on services provided by the IRU.
  • Ensures that there is an appropriate and available resource when he/she is unavailable to respond to referrals.
  • Supports sales and marketing through educational programs, tours, meetings, etc.
  • Develop strategies to grow business lines and market share in core hospitals-meeting census/occupancy targets.
  • Performs all other duties as assigned.

Minimum Job Requirements

  • Licensed clinician preferred (RN, LPN, OT, PT, ST, or SW)
  • Current driver’s license
  • Ability to multi-task, adapt quickly, and highly motivated and goal-oriented
