Position Description


The case manager works as part of the Integrated Dual Disorders ICM team to implement, monitor, and document services to individuals enrolled on this team from a trauma informed, DEI focused lens, connection to resources and peer supports, and demonstrate strong advocacy skills on participant’s behalf.


Case Management


The aim of this program is to assist referred individuals in accessing transitional housing, connecting to community based mental health or substance use treatment services, and resource gathering (food boxes, medical needs, etc.) to promote overall stabilization while navigating the legal system.

The ICM team provides comprehensive wrap around services through inter-disciplinary care to assist individuals to meet their basic needs, promote safety, accomplish goals, as outlined in the collaborative treatment plan, and attain the highest level of independence possible.

  • Engagement and Collaboration - providing skills training, advocacy, and community navigation
  • Case Management - coordinate with community resources to access food, housing, and medical care while providing assistance with navigating the judicial system
  • Crisis Prevention/De-escalation/Harm Reduction - provide crisis response and de-escalation in a community based setting; aim to decrease use of crisis services, EMS, police, hospitalization, etc
  • Supportive Skills Training - Provide direct or group skills training within transitional housing or community
  • Advocate for and with participant.
  • Monitor medication distribution, assist with prescriber visits.
  • Monitor, coordinate, and document all agency services


Qualifications:


A minimum of two years of experience in social work or related field preferred. Prior experience with SPMI, houseless population, and community based services preferred. Spanish/English desirable.

Applicant must possess the skills necessary to work independently. Understanding of psychiatric medications and their use, Administrative rules and other applicable laws and regulations, and thorough understanding of acceptable clinical practices and guidelines. Ability to obtain Oregon Driver’s license within 30 days of hire.

Qualified Mental Health Associate with a Bachelors Degree in social work or related field is required. Progress toward obtaining CADC I certification or willingness to attend classes and receive supervision toward certification.
