White Horse Village is seeking to add a Manager of Care Coordination to oversee our Outpatient department! The Manager of Care Coordination will plan, develop, and provide the leadership for the directing, coordinating, and implementing of the services of Outpatient Department. The Clinical Manager will be a full time position.

Duties:

  • The Manager of Care Coordination Oversees the Outpatient department with a team to provide healthcare services to residents in an active senior community.
  • The Manager of Care Coordination provides home visits to determine health care needs post hospitalization and post discharge from the health care center.
  • The Manager of Care Coordination provides complete home visits/assessments as needed.
  • The Manager of Care Coordination evaluates home visits and assessments to determine cognitive/functional changes that indicate the need for supportive services or a level of care change.
  • The Manager of Care Coordination responds to emergency calls and institutes appropriate emergency interventions as needed.
  • The Manager of Care Coordination conducts both formal and informal educational programs
  • The Manager of Care Coordination performs assessments based on residents’ needs and shares findings with the attending physician and team as appropriate.
  • The Manager of Care Coordination plans and implements interventions to meet these needs, utilizing internal and external resources, as appropriate.
  • The Manager of Care Coordination assists physicians in the screening of current and prospective residents.
  • The Manager of Care Coordination oversees medication exchange program for residents in need of this service.
  • The Manager of Care Coordination maintains resident healthcare records.
  • The Manager of Care Coordination develops positive relationships with residents and family members/significant others.
  • The Manager of Care Coordination performs quality improvement audits.
  • The Manager of Care Coordination monitors and audit various areas for quality assurance
  • The Manager of Care Coordination assists with Personal Care Home as needed for consultation and audits.
  • The Manager of Care Coordination works some required weekends on call

Qualifications

  • Must hold a valid LPN license
  • Minimum of 3 years in an outpatient/physicians office setting
  • Minimum of 3 years management experience
  • Personal Care experience a plus but not required
  • Excellent nursing judgement and assessment skills

Must be:

  • Vaccinated against COVID19 (will accept religious and/or medical exemptions)
  • Have an up-to-date physical
  • Have an up-to-date TB test
  • Have an up-to-date Flu Vaccination for year 2022.
  • Be able to pass a Pennsylvania Background Check

Job Types: Full-time, Part-time

Pay: $70,000.00 per year
